Citizens Memorial Hospital (CMH) in Bolivar, Missouri is recruiting a Community Health Equity Coordinator.

This is a new position responsible for managing CMH’s participation in MO HealthNet’s 5-year pilot project - Transformation of Rural Community Health (ToRCH). The ToRCH program directs resources to rural communities committed to addressing the ‘upstream’ causes of poor health through integrating social care supports into clinical care.

This position will serve as a primary community partner liaison and community agency network coordinator for implementing ToRCH-related activities. The ideal candidate will have a desire to collaborate with community partners to create regional social service networks to support systems for both micro and macro social needs for population health.

This position is full-time, days, Monday-Friday.

Job Requirements:

• Education: A Bachelor’s Degree in Social Work (BSW) is required.
• Experience: Prior experience serving in a community social services role is desired. Existing knowledge of community-based organizations and resources is preferred.
