Our facility is committed to ensuring superior quality of care and services in spacious, comfortable and conveniently located center. The Assistant Admission Coordinator will be responsible for facilitating admissions from hospitals on a daily basis. Included in establishing and maintaining positive relationships with Families, Case Managers, Social Workers, Discharge Planners, Insurance Case Managers and Physicians who will be involved in the placement of a patient to a skilled nursing facility (SNF). The Assistant Admission Coordinator will assist in the coordination of referral to admission conversion process as well as coordinating needed signed paperwork for admission to the facility. The successful candidate will use their expertise to coordinate evaluations of hospital patients to assess suitability for facility placement and acts as liaison with hospital discharge planners and other pertinent personnel. Experience in Long Term Care and/or Admission Process preferred
