CINQCAREPractice Transformation Manager Position DescriptionOverviewThe Practice Transformation Manager reports to the Practice Transformation Manager with accountability for providing strategy, judgment, organization, and evidenced-based analysis to influence decisions, and directly to meet CINQCARE requirements. They should embody CINQCARE’s core values, including, Trusted, Empathetic, Committed, Humble, Creative and Community-Minded. At CINQCARE, we don’t have patients or customers – we have Family Members.Primary Responsibilities The PTM will have the following responsibilities: 
  • Assist and support practices in optimizing available information technology resources to enhance patient outcomes and office efficiencies, covering areas.
 
  • Collaborate with interdisciplinary teams to identify opportunities for optimizing information technology resources to support healthcare practices, including:
    • Population health management strategies
    • Registry and enrolment functionality, including alignment with relevant programs.
    • Care coordination workflows to improve planned and walk-in care throughput.
    • Implementation of care management strategies to enhance patient care quality and outcomes.
    • Development of patient action plans and goal-setting processes to improve cost, quality, and patient experience.
    • Collection and reporting of outcomes data to monitor performance and drive improvements.
    • Implementation of patient safety protocols and initiatives
    • Facilitation of community learning forums to share best practices and foster collaboration.
 
  • Serve as the primary liaison and subject matter expert between healthcare organizations and external stakeholders, such as care management teams, to facilitate the integration of clinical processes and support tools.
  • Provide guidance and support to care teams, including physicians and staff, in adopting best practices, identifying performance gaps through data analysis, and implementing quality improvement strategies.
  • Build and maintain trust-based relationships with healthcare practices and care teams to facilitate change management, engagement, and sustainability of transformation initiatives.
  • Deliver progress reports to healthcare practices and care teams, identifying performance gaps and providing guidance on solutions.
 
  • Prioritize key quality and utilization initiatives critical to success under value-based care programs.
 
  • Lead or support practice transformation forums and meetings to facilitate knowledge sharing and collaboration among stakeholders.
 
  • Provide training and guidance on the use of technology tools to healthcare practice teams as needed, staying informed about updates and integrating new functionality into workflows.
 
  • Ensure compliance with regulatory guidelines and standards, including those set by NCQA and other relevant bodies.
 
  • Document and assess practice progression throughout the transformation process.
 
  • Facilitate efficient and effective improvement team meetings to drive continuous improvement efforts.
  • Connect healthcare practices with external resources, such as consultants and group learning opportunities, to support transformation efforts.
 
  • Coordinate interactions and follow-up actions with other functional areas within the healthcare organization, such as Network Management/Provider Relations, Medical Economics, and Health Home.
 
  • Drives success in population health performance by guiding care teams (practice physicians, staff) towards best practices, identifying gaps both through data and operational observation, by providing feedback, education, and training.
  • Obtains and analyzes (will they need to analyze???) practice specific cost and quality data and reports for cost of care and quality opportunities and assists practice to target high risk members and gaps in care based on data/reports provided.
  • Build trust-based relationships with practices and care teams to facilitate change management, engagement, solutioning, and sustainability of population health initiatives
  • Deliver progress reports and review with practices and care teams.
  • Identify performance gaps from report data and guide care teams towards solutions
  • Identify and prioritize key quality and utilization initiatives critical to success under performance-based programs
  • Co-facilitate and/or lead practice transformation forums and meetings related to value-based care initiatives.
  • Provide training and guidance on the use of company technology tools to practice care teams as necessary.
  • Stay current with updates on technology tools, and guide care teams in integrating new functionality into workflows.
  • Assure that standards of practice and policies follow NCQA and other regulatory guidelines and standards.
  • Deploy PDSA best practices from one practice to the next to improve operational and quality metrics.
  • Assess and document practice progression throughout the transformation process.
  • Facilitate efficient, effective improvement team meetings.
  • Link the practice to available external resources, such as consultants, web links, and group learning opportunities.
  • Coordinates, as appropriate, practice interactions and/or follow-up actions with other CINQ Care functional areas, including, but not limited to, Network Management/Provider Relations, Medical Economics, and Health Home.
General DutiesThe Practice Transformation Manager should have the following duties: 
  • Leadership: The Practice Transformation Manager will lead in defining and executing strategies and solutions to create business value in clinical, including building a team to design, develop, and execute those strategies and solutions to deliver desired outcomes. Serves as the subject matter expert and liaison with CINQ Care Management to facilitate clinical processes between the organization and the practice, creates and hosts tailored learning opportunities to support the deployment of program interventions, use of CINQ Care’s support tools and other data resources, and provides technical support for such tools, as required.
 
  • Strategy: The Practice Transformation Manager will establish the business strategy and roadmap: (1) improve outcomes for CINQ Care Inc. members; (2) enhance the efficacy of other CINQ Care Inc. business divisions; and (3) develop and deliver external market opportunities for CINQ Care products and services. In establishing the business strategy, the Practice Transformation Manager will define and innovate sustainable revenue models to drive profitability of the Company.
 
  • Collaboration: The Practice Transformation Manager will ensure that clinical capabilities form a cohesive offering, including by working closely with other business divisions to learn their needs, internalize their knowledge, and define solutions to achieve the business objectives of CINQ Care Inc.
 
  • Knowledge: The Practice Transformation Manager will provide subject matter expertise in clinical solutions, including determining and recommended approaches for practice transformation solution deployment and performance evaluation.
 
  • Culture: The Practice Transformation Manager is accountable for creating a productive, collaborative, safe and inclusive work environment for the clinical team and as part of the larger Company.
Qualifications The Practice Transformation Manager should have the following qualifications: 
  • Education. BA/BS, master’s in healthcare or related field and/or clinical license (RN, LPN).
 
  • Experience. Ideal candidates will have 5 years of relevant practice experience in the healthcare industry; Previous experience in healthcare settings, particularly in primary care practices with successful implementation of quality improvement initiatives, is advantageous. Coursework or experience in Quality Improvement, and/or facilitation skills preferred. Qualified experience in healthcare settings preferred. Ideal candidate will have had experience in a primary care practice that successfully implemented quality improvement initiatives.
 
  • Entrepreneurial. CINQ seeks to fix gaps that have persisted for generations in the delivery of care to Black and Brown populations. Excellent verbal, written communication and presentation skills; ability to clearly articulate and present concepts and models in an accessible manner to CINQ’s team, investors, partners and other stakeholders. Proficiency in all Microsoft Office applications; Ability to interact professionally with diverse stakeholders, including healthcare facilities, staff, and community partners. Requires strong interpersonal communication skills, excellent verbal. written, presentation, and group facilitation skills. Must have strong critical thinking skills necessary to evaluate and respond to team progress, anticipate needs and other related team dynamics.
 
  • Relationships. Ability to build and effectively manage relationships with business leaders and external constituents; and, Ability required to interact with multiple facilities, staff, and community partners in support of CINQ Care’s Core Values of integrity (honesty & protection of confidentiality), scholarship (contribute to dissemination), teamwork (help others), interdisciplinary collaboration (contribute to partnerships), (foster advancement of the team). Ability to execute with a sense of urgency is required.
