Community Health Worker
Position Description
Overview
The Community Health Worker reports to the Community Health Worker Supervisor with accountability for providing strategy, judgment, organization, and evidenced-based analysis to influence decisions, and directly to meet CINQCARE requirements. They should embody CINQCARE's core values, including, Trusted , Empathetic, Committed, Humble, Creative and Community-Minded . At CINQCARE, we don't have patients or customers - we have Family Members.
The Community Health Worker (CHW) will provide non-clinical participant outreach and engagement, screening and assessment, coordination of non-clinical services and supports, family and psychosocial support and health promotion coaching, and monitoring of participant wellbeing throughout the perinatal period.
Job Responsibilities
The Community Health Worker will have the following responsibilities:

• Complete all required HRSA HS trainings
• Conduct outreach, engagement, and initial screening/assessment with prospective participants
• Coordinate non-clinical services and supports listed in the participant's plan of care Process prospective HS participant referrals and dispatch CHWs to conduct initial screening and enrollment
• Provide family and psychosocial support and health promotion coaching
• Enter case notes and supporting documentation into participant case files
• Monitor general wellbeing of mom, baby, and family throughout the perinatal period
• Support participant transitions of care as directed by RN-CM
• Support participant participation in HS project group-based health education
• Participate in daily huddles and weekly case conferences
• Maintain lines of referral across SDOH and other social service partners/providers

General Duties
The Community Health Worker will have the following duties:

• Leadership : The Community Health Worker will lead in defining and executing strategies and solutions to create business value, including building a team to design, develop, and execute those strategies and solutions to deliver desired outcomes.
• Strategy : The Community Health Worker will establish the business strategy and roadmap: (1) improve outcomes for CINQCARE Family Members; (2) enhance the efficacy of other CINQCARE. business divisions; and (3) develop and deliver external market opportunities for CINQCARE products and services. In establishing the business strategy, the Community Health Worker will define and innovate sustainable revenue models to drive profitability of the Company.
• Collaboration : The Community Health Worker will ensure that capabilities form a cohesive offering, including by working closely with other business divisions to learn their needs, internalize their knowledge, and define solutions to achieve the business objectives of CINQCARE.
• Knowledge : The Community Health Worker will provide subject matter expertise in solutions, including determining and recommended approaches, solution deployment and performance evaluation.
• Culture : The Community Health Worker is accountable for creating a productive, collaborative, safe and inclusive work environment for the team and as part of the larger Company.

Qualifications
The Community Health Worker should have the following qualifications:

• Education: Associate's degree or equivalent in human services, public health, social work or similar field with a record of strong academic achievement;
• Experience: At least 1 year case management/care coordination experience in greater Buffalo;

• Must be from and representative of the served community
• Comfort working primarily in the community in a diverse, fast-paced environment
• Strong, practiced familiarity with Buffalo-area public assistance programs, social service providers, and community-based organizations
• Experience with electronic medical records preferred
• Experience working as part of a diverse team

• Communication: Excellent verbal, written communication and presentation skills; ability to clearly articulate and present concepts and models in an accessible manner to CINQCARE's team, investors, partners, and other stakeholders. Proficiency in all Microsoft Office applications.

Location : Buffalo, NY
Compensation: $20 - $24/hr
