CINQCARECommunity Consortium CoordinatorOverviewPosition DescriptionThe Community Consortium Coordinator reports to the Healthy Starts for Buffalo Project Director with accountability for providing strategy, judgment, organization, and evidenced-based analysis to influence decisions, and directly to meet CINQCARE requirements. They should embody CINQCARE’s core values, including, Trusted, Empathetic, Committed, Humble, Creative and Community-Minded. At CINQCARE, we don’t have patients or customers – we have Family Members.Job ResponsibilitiesThe Community Consortium Coordinator will have the following responsibilities:
  • Coordinate Consortium communication, meetings, work groups, and products (including distribution of those products across the region and among stakeholders)
  • Facilitate Consortium completion of community needs assessment and environmental scan, in collaboration with CINQCARE’s analytics team, to identify potential SDOH and other needs that drive adverse perinatal outcomes in the area
  • Draft a Consortium Action Plan, using a participatory process that engages Consortium Members
  • Support Consortium Members’ implementation of the Action Plan
  • Coordinate Consortium Members’ advice and support for HS staff and partners
  • Manage the HS project website and associated blog, listserv, and other public engagement tools to increase community awareness of project objectives, progress, data, events, and improvements to the perinatal system in Buffalo
  • Communicate with and leverage the capacities of the Project Director and CINQCARE leadership for Consortium success
  • Ensure Consortium participation in HRSA’s defined Communities of Practice with other HS and HSE projects to develop and implement a community-level plan that addresses one or more SDOH that impacts perinatal health in the project area
General DutiesThe Community Consortium Coordinator Will Have The Following Duties
  • Leadership: The Community Consortium Coordinator will lead in defining and executing strategies and solutions to create business value, including building a team to design, develop, and execute those strategies and solutions to deliver desired outcomes.
  • Strategy: The Community Consortium Coordinator will establish the business strategy and roadmap: (1) improve outcomes for
  • Collaboration: The Community Consortium Coordinator will ensure that capabilities form a cohesive offering, including by working closely with other business divisions to learn their needs, internalize their knowledge, and define solutions to achieve the business objectives of CINQCARE.
  • Knowledge: The Community Consortium Coordinator will provide subject matter expertise in the solutions, including determining and recommended approaches, solution deployment and performance evaluation.
  • Culture: The Community Consortium Coordinator is accountable for creating a productive, collaborative, safe and inclusive work environment for the team and as part of the larger Company.
Qualifications The Community Consortium Coordinator Should Have The Following Qualifications
  • Bachelor’s degree in public health, community development, or equivalent with a record of strong academic achievement;
  • Experience: At least 5 years’ stakeholder and community building experience;
  • Excellent written and verbal communication skills and experience liaising with diverse stakeholders about issues of common concern
  • Must be from and representative of the target population in the project area
  • Familiarity with U.S. Centers for Disease Control and Prevention Community Needs Assessment Participant Workbook, preferred
  • Entrepreneurial: CINQCARE seeks to fix gaps that have persisted for generations in the delivery of care to Black and Brown populations. This position is accountable for ensuring CINQCARE is positioned to innovatively deliver on its promise.
  • Communication: Excellent verbal, written communication and presentation skills; ability to clearly articulate and present concepts and models in an accessible manner to CINQCARE’s team, investors, partners, and other stakeholders. Proficiency in all Microsoft Office applications.
  • Relationships: Ability to build and effectively manage relationships with business leaders and external constituents; and,
  • Good judgement, impeccable ethics, and a strong team player; desire to succeed and grow in a fast-paced, demanding, and entrepreneurial Company.

Compensation: $67K – $76K
