The Social Services Liaison provides training, education and oversight to Ciena Healthcare facilities Social Services department. Services provided meet professional standards of social work practice, consistent with state and federal laws and regulations.

Responsibilities:

  • Demonstrate assessment skills sufficient to evaluate residents’ behavior, to collect data to evaluate psycho-social needs, risk factors for psycho-social deterioration and residents’ responses to interventions. Able to interpret and explain resident behavior to staff in way that fosters understanding, facilities treatment and respect for resident rights and minimizes use of chemical and physical restraints.
  • Able to implement social service interventions that achieve treatment goals, address resident needs, link social supports, physical care and physical environment to enhance quality of life.
  • Management skills sufficient to oversee the organization of facility department staff, to allocate resources and to supervise interdisciplinary cooperation in the planning an implementation of individual resident plans of care.
  • Interpret and assure implementation of governing board policies and procedures.
  • Interpret and oversee the implementation of facility policies and procedures in compliance with corporate, state, federal and other regulatory guidelines.
  • Ensures that the quality and appropriateness of patient care meets or exceeds company and industry standards and ensures that all services are in compliance with state and federal legal, regulatory, accreditation and reimbursement guidelines.
  • Supports program development activities with the marketing staff for the purpose of introducing new and relevant services for the persons served and the community.
  • Attend and/or conduct facility meetings, as required to carry out responsibilities.
  • Promotes optimal communication among staff members, facility staff, referrals sources, physicians and patients.
  • Seeks out external resources through conferences, workshops, etc. as necessary. Routinely shares personal knowledge with staff, other managers and facility personnel.
  • Oversee Activity Department and makes recommendations.
  • Assure adequate preparation for, and participate in regulatory compliance surveys.
  • Participates in quality improvement activities, which reduce cost and increase efficiency through improved systems and processes.
  • Interprets Company policy and makes decisions based on the application of those policies where applicable.
  • Provides training and education to facility Social Work employees.
  • Monitors and tracks Home Health referrals.
  • Serves as a supervisory for staff that are in the process of obtaining their license.

Requirements:

  • Minimum of Bachelor’s Degree in Social Work or in a human services field (including but not limited to Sociology, Special Education, Rehabilitation Counseling and Psychology). Master’s Degree preferred and licensed.
  • Five years supervised social work experience working directly with individuals in a Long Term Care.
