POSITION SUMMARY:

The Wellness Nurse is responsible for working to ensure that resident needs are met throughout the shift. Duties include processing, verifying and approving provider orders, working with Pharmacy, communicating with residents, internal and external providers, families, visitors and vendors. Provides education to unlicensed personnel and performs delegation duties as appropriate (RN only). 

ESSENTIAL JOB FUNCTIONS: 

· Supports the mission, vision, and goals of the organization, upholding and promoting company culture and vision. 

· Observe and record patient behavior. 

· Assist in the direction, coordination and evaluation of wellness services provided to residents. 

· Perform resident evaluations/assessments as appropriate/required.

· Collect resident health history.

· Counsel and educate residents and their families.

· Resolve/elevate resident care issues/concerns. 

· Administer medication per provider orders and provide hands-on care as needed per the individualized service plan.

· Read and interpret resident information, make treatment decisions when appropriate, elevating to HWD, regional/home office resources or providers when appropriate. 

· Supervise, direct and provide education to medication aides and caregivers. 

· Ensuring compliance with all regulatory guidelines. 

· Ensuring all confidentiality and privacy rights are observed and enforced. 

· Perform RN delegation tasks as appropriate and in accordance with regulatory guidelines. 

· Attends training courses at community and maintains up to date monthly, quarterly, annual training through designated Learning Management System (LMS).

· Other duties as assigned, and which relate to the success of the community and the care, comfort, and happiness of our residents.

Requirements

EXPERIENCE, QUALIFICATIONS & SKILLS: 

· Licensed as a Registered Nurse and in good standing with the Board of Nursing. 

· Must have excellent computer skills.

· Excellent written and verbal communication skills. 

· Able to follow all regulatory guidelines.

· Must have strong understanding and command of the English language, sufficient to read, write, interpret and communicate effectively with residents, family members, fellow team members, vendors and other visitors. 

PHYSICAL DEMANDS: 

The following physical demands are representative of those that must be met to successfully perform the essential functions of this position: 

· Walk/Stand – must be able to continuously walk and stand.

· Environment Condition – must be able to perform work both inside and outside.

· Sit – sit infrequently.

· Lift - constantly 0-10 pounds, frequently 11 – 50 pounds, occasionally 51 – 100 pounds.

· Carry - constantly 0-10 pounds, frequently 11 – 50 pounds, occasionally 51 – 100 pounds.

· Push - constantly 0-10 pounds, frequently 11 – 50 pounds, occasionally 51 – 100 pounds.

· Climb – must be able to climb stairs on a frequent basis, with the opportunity to utilize the elevator as an alternate resource if available.

· Bend – must be able to bend at the waist, knees, hips, and spine on a frequent basis; may require frequently manipulating weights of 25 pounds or more.

· Squat – must be able to squat on a frequent basis to reach below the knee, may require lifting of 50 pounds or less.

· Twist or rotate – must be able to twist or rotate occasionally at the trunk, hips, knees, and neck, manipulating weights of 25 pounds or more.

· Reach– must be able to reach on a frequent basis. May occasionally reach overhead, requiring manipulation of weight of 10 pounds or less, and below waist manipulating weight of 25 pounds or more.
