Per Diem MDS Coordinator

Seabury MDS Coordinator are highly rewarded! We are searching for enthusiastic, caring team members for our continuing care community! Rates begin at $45-$50 per hour .

Position Requirements:  

EDUCATION

  • Associates or Bachelors degree with a Valid Nursing license as RN with an accredited institution, listed with the Connecticut State Registry.
  •  2 or more years of education in rehabilitative and geriatric nursing, or equivalent experience.

EXPERIENCE

  • 2 or more years of MDS and Patient Care Planning
  • Knowledge of OBRA Regulations

Skills and Competencies: 

  • Ability to read, speak and understand the English language clearly.
  • Strong interpersonal, organizational, and communication skills in person and via telephone. 
  • Ability to make decisions and assessments.
  • Strong computer skills; knowledge of MatrixCare and Microsoft Office helpful.
  • Ability to maintain good rapport with staff, families, physicians and residents.
  • Ability to work calmly and efficiently under pressure.
  • Ability to relate to elderly in a dignified manner.
  • Document Medicare Certifications and Recertification’s ensuring doctor’s signatures are present.
  • Submitting Medicare Denial Letters to Resident/Power Of Attorney (POW)/Health Care Advocate (HCA) per Medicare regulations.
  • Facilitation skills for conducting Care Conferences.
