Nurse Practitioner - Geriatric Primary Care

- $100,000 to $120,000 Base Salary, dependent upon experience

COMPANY PROFILE: 
This organization is a leading provider of integrated medical services, caring for individuals from post-hospitalization to home in various settings - skilled nursing facilities (SNFs), assisted living facilities (ALFs), independent living facilities (ILFs), group homes, and private residences. They bring quality medical care to the geriatric population, as well as people with complex conditions, special needs, and intellectual and/or developmental disabilities. Their providers make a dramatic impact in increasing quality of life and safeguarding the dignity of those they serve.

POSITION DESCRIPTION: 
Serve as the primary care provider for a dedicated panel of 200 to 225 geriatric patients.

The practice believes strongly in proactive medicine, with a clinical focus on:

- preventative care
- chronic disease management
- medication management
- routine geriatric primary care

Providers typically see patients once a month, but this is adjusted per provider request based on patient needs.

Providers typically travel to 2 buildings each day and see 10 to 11 patients, but each day varies and some days involve house calls. Providers perform 50 to 55 visits total per week, with each day structured by a talented scheduler to account for travel time, types of visits scheduled, etc.

The practice uses a mobile-based EHR specialized to this type of setting.

Providers share on-call responsibilities, which is phone triage only and light volume. Full-time providers are typically on call no more than 3 nights per month and some flexibility is offered.

REQUIREMENTS:
- Virginia NP license
- DEA
- 1 year of advanced practice experience
- previous experience in home care, geriatrics, or a relevant area
