Description

PAY RATE: $15.11 Hourly (Pay is negotiable based on experience) 

Provides home visits, transports or occasional In-home respites for individuals who may be developmentally, physically and/or mentally challenged. Hours of service depend on the needs of the participant. Leads Caseworker I and long-term Volunteers with training and provides support for GoCanvas, scheduling and other aspects of home visitation component. Assists Coordinator with monthly paperwork and other duties as time allows.

Prioritize participants needs to provide home visits, transports or occasional In-home respites as scheduled. Assume Elderly Home Repair (EHR) paperwork as needed.

Lead Caseworker I and long-term Volunteers in various aspects of home visitation component. Provides support for coordinator as time allows.

Submits completed paperwork and assists with the planning & preparation of program special events.

Qualifications

Requirements: 

High School Diploma or Equivalent Required

Certificate In a Related Field Required

18 Months Additional Training In a Related Field Required

3 or more years of successful experience in related field or combination of education and experience

Possess a Valid Kentucky Driver's License and Maintain a Safe Driving Record Required

Kentucky CAP Driving Certification (or Ability to Obtain) Required

CNA - Kentucky Certified Nursing Assistant Required 

Physical Requirements: 

Ability to reach, push, pull, bend, stoop, stand, walk

Lift 50 lb and carry 20 ft

Push cart with 10 lb of force 30 ft

Lifts/moves items with unknown weight or that are awkward shapes and sizes with assistance

Have good visual acuity with corrective lenses

Have good hearing capabilities
