CHP Home Care & Hospice is Hiring a Hospice LPN Near Defiance, OH

PRN/Part-time and potentially full-time LPNs needed for inpatient hospice and home health and hospice care. Provide excellent nursing care in your community. Work one-on-one with local patients and families.

*What You Will Do*A Hospice LPN (Licensed Practical Nurse) plays a crucial role in providing compassionate care to terminally ill patients and their families. Deliver one-on-one care, including monitoring vital signs, administering medications, and providing emotional support. Patients may include those who are chronically ill, disabled, or elderly, individuals recovering from an accident or injury, and the terminally ill.
*Job Responsibilities
  • Monitoring patient vital signs, including body temperature, heart rate, respiration rate and blood pressure
  • Administering medications and treatments prescribed by physicians
  • Notifying physicians when they observe a change in a patient's condition
  • Counseling family members to help them know what to expect as their loved one's condition worsens
  • Making family members aware of resources like counseling and government programs
  • Helping family members complete paperwork
  • Updating patient records
  • Adhering to infection control, standards of care, and patient privacy standards
*Skills*Home health and hospice LPNs use a variety of skills and clinical knowledge to provide the most comprehensive care.
  • Observation skills to recognize changes in patients who may be unable to communicate
  • Strong oral communication skills to direct other members of the healthcare team and deliver reports to physicians
  • Well-developed interpersonal skills to interact positively with patients and family members
  • Attentiveness to detail to maintain accurate patient records
  • Knowledge of the standards of care for end-of-life care
  • Familiarity with pain relief drugs and other medications and treatments commonly prescribed for hospice patients
  • Computer skills, including using email and patient record management software
*Qualifications
  • Proven nursing experience
  • Driver's license and reliable, insured, vehicle
  • Valid Ohio nursing license

Pay:
From $28.86 per hour
Expected hours: 12 – 40 per week
