Job Description:


Job Summary

Chloeta is seeking an Urgent Care/ER Family Nurse Practitioner or Physician's Assistant (FNP/PA) to provide patient care at a medical facility in Chinle, AZ. Chinle provides medical care for approximately 35,000 Navajo. Serving a rural area, many of our patients travel 100 miles round trip to receive care and do not have electricity or running water in their homes. In addition, most of our elderly patients speak only Navajo, and live according to traditional Navajo cultural practices.
A collegial group of providers offer services in primary care, women's health, counseling, dental, dietary and community nutrition, health promotions, lab, x-ray, optometry, pharmacy, physical therapy, speech therapy, podiatry, and public health nursing. When not providing care in the clinic, the group enjoys book clubs, potlucks, gardening, fire pits, learning about the Navajo culture, hiking, rock climbing, camping, biking, cross country skiing, downhill skiing, kayaking rock climbing, fishing and national parks are all within a few hours away.

Duties/Responsibilities

  • Interviews and examines patients, reviews past medical history, and requests and/or performs complex diagnostic tests and examinations deemed necessary to obtain all possible information related to each case.
  • Makes preliminary diagnosis, directs, prescribes, or provides treatment, or arranges for specialized care or patient referral as required.
  • Evaluates patients within the full age spectrum presenting for services. Bases decisions upon information obtained, professional medical knowledge and skills and prescribe policies and procedures. Makes note of observations to be incorporated into clinical records.
  • Gives total patient care including prevention, health maintenance, early diagnosis, treatment, and follow-up services to patients under his/her care. Obtains consultation as needed. As clinically appropriate, provides medical care to patients for wide spectrum of acute and chronic illness.
  • May serve as a member of the code team in life threatening situations for which the “code blue” or “medical emergency” activation system has been initiated.
  • When working in urgent care department, provides patient care to individuals within the full age spectrum and obtains consultants as needed. Patients occasionally present to urgent care with urgent medical issues and may require prompt evaluation and decision making. Provides urgent care including evaluation, stabilization and transfer of patients with sepsis, MI, laceration, fracture, abdominal pain, glycemic issues, psychiatric illness, respiratory conditions, pediatric illness and renal dysfunction.
  • Recognizes and gives initial treatment of acute and psychiatric problems.
  • Provides primary medical care to patients with a wide spectrum of illness.
  • Provide telemedicine services for patients with mild respiratory illnesses.
  • Responsible for utilizing the EHR system to obtain, dictate, electronically enters and assures preparation of appropriate medical records for all patients seen to assure the accumulation and organization of all pertinent clinical data needed to provide comprehensive medical care. Clinical data reports shall be completed in a timely manner and shall be maintained in compliance with Joint Commission standards.
  • Conducts outpatient clinics making observations appropriate to the outpatient’s underlying disease and treatment plan, including current symptoms. Reviews the patient’s medications, diet, therapy course, adherence to management plan and patient observations. Modifies regimen as required. Instructs and counsels patients as required.
  • Refers patients to appropriate contracted medical or other facilities, providing full clinical information for care and diagnostic procedures that cannot be adequately provided at local facilities.
  • Other duties as assigned.

Education Qualifications

  • Required: The FNP or PA must have completed an accredited program in the U.S.A. with a minimum of six (6) months experience in the specialty required by the contract. There must be no lapse of services of more than six (6) consecutive months for at least six (6) months prior to beginning tour of duty.
  • Required: Able to commit to a minimum of 4 weeks or more at a time.
  • Required: Comfortable with seeing an average of 25-30 patients per shift.
  • Required: Be board certified as NP or PA.
  • Required: Must possess a current, valid, unrestricted, applicable medical license in any state, the District of Columbia, the Commonwealth of Puerto Rico, or a Territory of the United States, throughout the term of this contract.
  • Required: Must maintain a valid, current, full, and unrestricted DEA license with schedule types II through V and maintain certification throughout the term of the contract. License verification must be provided at the time of application.
  • Required: Shall maintain the following valid and current certifications:
    • Cardiopulmonary Resuscitation (CPR),
    • Basic Life Support (BLS),
    • Advanced Cardiac Life Support (ACLS),
    • Advanced Trauma Life Support (ATLS), and
    • Pediatric Advanced Life Support (PALS).
  • Required: Provide 3 professional references.

Physical Requirements

  • Must be able to withstand physical and emotional stress.
  • Requires long hours of sitting, standing and moving.
  • Able to frequently lift at least 50 lbs., reach, pull, push, bend, stoop, and kneel.
  • At the time of hire, must provide a fitness for duty certificate issued by a licensed physician to perform the proposed job without significant risk to personal health or the health and safety of others.
  • At the time of hire, must provide documentation of evidence of immunizations as required by the facility's immunization policy.

Pre-employment Requirements:

This position may be contingent upon passing a security clearance, criminal background check (state and federal), drug screen, Motor Vehicle Record (MVR) review, education verification, employment verification, and/or credit check after acceptance of an offer and prior to beginning work, due to the nature of the services Chloeta provides and the responsibilities of this position.

Drug and Alcohol Testing:

To enforce Chloeta’s Drug and Alcohol Testing Policy, an employee or candidate could be subject to undergoing a drug screen and/or alcohol testing at the beginning of employment, as well as after commencement of employment where there is reasonable suspicion of intoxication, or following an automobile or other accident, or as part of a company or facility-wide random drug screen process.

Fair Credit Reporting Act (FCRA):

Under the FCRA, Chloeta may request a credit check during the onboarding process. A written authorization form will be required before obtaining credit records.

Emergency Response:

Every Chloeta employee has regular and recurring emergency management responsibilities, though not every position requires routine deployment to disaster sites. All positions are subject to recall around the clock for emergency management operations, which may require irregular work hours, work at locations other than the official Duty Station, and may include duties other than those specified in the employee's official Position Description (PD). Travel requirements in support of emergency operations may be extensive in nature (weeks to months), with little advance notice, and may require employees to relocate to emergency sites with physically austere and operationally challenging conditions.
