Job Description:


Job Summary:

Chloeta is seeking four Public Health Nutritionists for a medical facility in Shiprock, NM. The Public Health Nutritionist will provide education to patients with a variety of nutrition related health conditions such as, obesity, hypertension, and hyperlipidemia. A minimum one (1) year commitment is preferred. Options for additional consecutive one (1) year commitments do typically exist but are based on client needs.

Supervisory Responsibilities: None

Duties/Responsibilities:

  • Provides Medical Nutrition Therapy (MNT) and diabetes self-management education to patients with diagnoses of pre-diabetes, diabetes or diabetes in pregnancy with educational visits that are based on individual assessments, self-care plans and evaluation of personal goal setting in individual or group settings.
  • Documents in EHR educational assessment, topics covered and evaluation of patient's level of understanding and personal self-care plan and evaluation of attainment of patient self-determined goals.
  • Participates in Diabetes Self-Management Education (DSME) for newly diagnosed patients and prediabetes for individuals and/or groups as an instructor and case manager for the program.
  • Participates in team and case management meetings for the Diabetes Model program.
  • Provides all aspects of diabetes, diabetes prevention education in outpatient, community, and home settings.
  • Provides direct care / services to pediatric, geriatric, adolescent, and adult patients.
  • Plans, implements, and participates in nutrition education, diabetes prevention and health promotion activities in the communities served by the diabetes program.
  • Assist Diabetes Program Director with the Special Diabetes Program grant application for work plan development and annual grant reporting for the interim and final progress reports.
  • Plans, develops, implements, and evaluates the nutrition component of the Diabetes Education Program in coordination and with the collaboration of other members of the team.
  • Reviews, selects, and develop educational materials and resources to ensure cultural appropriateness and sensitivity based on the client's educational needs along with team members.
  • Provides consultation, training, and technical assistance with the diabetes program staff on planning, implementing and evaluating diabetes prevention/health promotion programs and activities as needed.

Education/Experience Requirements:

  • Required: Minimum of 2 years of experience in a healthcare setting working with diabetes patients.
  • Required: Bachelor’s degree in Nutrition Sciences, Health Education, or related field from USA accredited school.
  • Required: Licensed to practice as a LD (Licensed Dietitian) or LN (Licensed Nutritionist) in the US. All providers shall possess a current, valid, unrestricted medical license in a state, the District of Columbia, the Commonwealth of Puerto Rico, or a Territory of the United States, throughout the term of this contract.
  • Required: Completion of a CDE (Certified Diabetes Educator) certification, or enrollment in a Certified Diabetes Care and Education Specialist credential program within 12 months of hiring date.
  • Required: Public Health/Community Health experience.
  • Required: Valid and current Cardiopulmonary Resuscitation (CPR), Basic Life Support (BLS) and Advanced Cardiac Life Support (ACLS).
  • Preferred: Master’s Degree in Nutrition Sciences or related field.
  • License verification and certifications must be provided at the time of application and can be emailed to certification@chloeta.com.

Physical Requirements:

  • Work environment involves possible exposure to hazardous materials, such as chemical, blood and blood borne pathogens.
  • Must be able to lift 50 pounds at times and be able to bend, lift and pull.
  • This position requires extended periods of being on your feet and prolonged work sitting at a desk operating a computer.
  • At the time of hire, must provide a fitness for duty certificate issued by a licensed physician to perform the proposed job without significant risk to personal health or the health and safety of others.
  • At the time of hire, must provide documentation of evidence of immunizations as required by the facility's immunization policy.

Pre-employment Requirements:

This position may be contingent upon passing a security clearance, criminal background check (state and federal), drug screen, Motor Vehicle Record (MVR) review, education verification, employment verification, and/or credit check after acceptance of an offer and prior to beginning work, due to the nature of the services Chloeta provides and the responsibilities of this position.

Drug and Alcohol Testing:

To enforce Chloeta’s Drug and Alcohol Testing Policy, an employee or candidate could be subject to undergoing a drug screen and/or alcohol testing at the beginning of employment, as well as after commencement of employment where there is reasonable suspicion of intoxication, or following an automobile or other accident, or as part of a company or facility-wide random drug screen process.

Fair Credit Reporting Act (FCRA):

Under the FCRA, Chloeta may request a credit check during the onboarding process. A written authorization form will be required before obtaining credit records.

Emergency Response:

Every Chloeta employee has regular and recurring emergency management responsibilities, though not every position requires routine deployment to disaster sites. All positions are subject to recall around the clock for emergency management operations, which may require irregular work hours, work at locations other than the official Duty Station, and may include duties other than those specified in the employee's official Position Description (PD). Travel requirements in support of emergency operations may be extensive in nature (weeks to months), with little advance notice, and may require employees to relocate to emergency sites with physically austere and operationally challenging conditions.
