Job Description:


Job Summary:

The Phlebotomist will provide phlebotomy services to a facility located in Fort Duchesne, UT. This position is ideally for one year with 4 options to extend.

Job Classification: Full-Time

Supervisory Responsibilities: None

Duties/Responsibilities:

  • Collect blood samples from patients with efficiency and centrifuge blood samples.
  • Assist the Physician, Physician Assistant, and/or Nurse with patient flow in a clinic setting by measuring the patient's vital signs and weight and scanning laboratory results in the Bureau Electronic Medical Record (BEMR).
  • Organize, schedule, and perform laboratory studies, specimen collection and process within the Health Service Unit, to include A&O in-processing, DNA per procedure, vaccine programs and PPD updates.
  • Prepare and ensure specimens are packed and transported according to the contract or Reference Lab and document per Health Services Unit Laboratory Procedures.
  • Ensure that lab requests are completed properly and all reports are returned from the contract lab, provider notified, log results in as received, follow up appropriately, and file results in lab and medical record.
  • Acquire and maintain knowledge of the Health Services Manual and Unit Procedures.
  • Ensure that the Laboratory Procedure Manual is updated accordingly.
  • Assist the Health Service Administrator (HSA) or his/her designee in the coordination of facility and Contract Reference Lab contact, CLIA re-certification, maintenance of equipment and notification of needed supplies.
  • Assist the HSA and Clinical Director with writing and implementing policies that deal with the laboratory program.
  • Performs other duties as assigned.

Education and Experience Requirements:

  • Required: Graduate from an accredited phlebotomy program or technical school.
  • Required: BLS certified.
  • Required: Provide three professional references.
  • Required: Submit a resume and requested documentation along with copy of all diplomas and/or certificates to support stated qualifications.
  • Required: Must provide proof of experience with specimen collection, packaging and shipment to referral laboratories with knowledge of maintaining appropriate records and proof of experience of working in a correctional setting.
  • Required: Have and maintain valid driver’s license or ID in any state of the U.S.A.
  • License verification and certifications must be provided at the time of application and can be emailed to certification@chloeta.com.

Physical Requirements:

  • May require long hours of sitting and performing computer work. However, work may demand frequent standing, walking, sitting, bending, carrying of items such as records, supplies, and working with equipment.
  • Able to lift at least 50 lbs., reach, pull, push, bend, stoop, and kneel.
  • At the time of hire, must provide a fitness for duty certificate issued by a licensed physician to perform the proposed job without significant risk to personal health or the health and safety of others.
  • At the time of hire, must provide documentation of evidence of immunizations as required by the facility's immunization policy.

Pre-employment Requirements:

This position may be contingent upon passing a security clearance, criminal background check (state and federal), drug screen, Motor Vehicle Record (MVR) review, education verification, employment verification, and/or credit check after acceptance of an offer and prior to beginning work, due to the nature of the services Chloeta provides and the responsibilities of this position.

Drug and Alcohol Testing:

To enforce Chloeta’s Drug and Alcohol Testing Policy, an employee or candidate could be subject to undergoing a drug screen and/or alcohol testing at the beginning of employment, as well as after commencement of employment where there is reasonable suspicion of intoxication, or following an automobile or other accident, or as part of a company or facility-wide random drug screen process.

Fair Credit Reporting Act (FCRA):

Under the FCRA, Chloeta may request a credit check during the onboarding process. A written authorization form will be required before obtaining credit records.
