CHLOETA is Hiring a Nurse Practitioner or Physician Assistant (NP/PA) Near Aberdeen, SD

Job Description:


Job Summary:

This position is for a Nurse Practitioner or Physician’s Assistant who shall provide medical care at a medical facility in South Dakota and North Dakota.

Supervisory Responsibilities: None

Duties and Responsibilities:

  • Interviews and examines patients, reviews past medical history, and requests and/or performs complex diagnostic tests and examinations deemed necessary to obtain all possible information related to each case.
  • Makes preliminary diagnosis, directs, prescribes, or provides treatment, or arranges for specialized care or patient referral as required.
  • Assists with Flu Vaccine off-site clinic.
  • Evaluates patients within the full age spectrum presenting for services. Bases decisions upon information obtained, professional medical knowledge and skills and prescribe policies and procedures. Makes note of observations to be incorporated into clinical records.
  • Manages all patient information in accordance with HIPAA standards, Privacy Act, Joint Commission standards, CMS standards, and Service Unit and/or Health Center specific policies and protocols.
  • Refers patients to appropriate contracted medical or other facilities, providing full clinical information for care and diagnostic procedures that cannot be adequately provided at local facilities.
  • Other duties as assigned.

Required Skills and Abilities:

  • Excellent verbal and written communication skills.
  • Possess knowledge, skills, and abilities to use a computer and navigate various platforms.
  • Cultural sensitivity.

Education and Experience:

  • Required: Completed an accredited Nurse Practitioner program or Physician Assistant program.
  • Required: Thirty-six (36) months of family practice experience, with a minimum of twelve months (12) in the specialty.
  • Preferred: Board certified with the American Nurses Credentialing Center or the American Association of Nurse Practitioners or the National Commission on Certification of Physician Assistants that is appropriate to specialty.
  • Required: Must possess a current, valid, unrestricted appropriate license in any state, the District of Columbia, the Commonwealth of Puerto Rico, or a Territory of the United States, throughout the term of this contract.
  • Required: Must maintain a valid, current, full, and unrestricted DEA license with schedule types II through V and maintain certification throughout the term of the contract. License verification must be provided at the time of application.
  • Required: Shall provide three (3) current, professional letters of reference from other health care providers familiar with the contractor's clinical practice skills within the same discipline.
  • Required: Shall maintain the following valid and current certifications:
    • Cardiopulmonary Resuscitation (CPR),
    • Basic Life Support (BLS),
    • Advanced Cardiac Life Support (ACLS), and
    • Pediatric Advanced Life Support (PALS).
  • Required: Must have and maintain a current driver's license throughout the contract period.
  • Certifications and licenses are due at the time of application and can be emailed to certification@chloeta.com.

Physical Requirements:

  • Must be able to withstand physical and emotional stress.
  • Requires long hours of standing and moving.
  • Able to frequently lift at least 50 lbs., reach, pull, push, bend, stoop, and kneel.
  • At the time of hire, must provide a fitness for duty certificate issued by a licensed physician to perform the proposed job without significant risk to personal health or the health and safety of others.
  • At the time of hire, must provide documentation of evidence of immunizations as required by the facility's immunization policy.

Pre-employment Requirements:

This position may be contingent upon passing a security clearance, criminal background check (state and federal), drug screen, Motor Vehicle Record (MVR) review, education verification, employment verification, and/or credit check after acceptance of an offer and prior to beginning work, due to the nature of the services Chloeta provides and the responsibilities of this position.

Drug and Alcohol Testing:

To enforce Chloeta’s Drug and Alcohol Testing Policy, an employee or candidate could be subject to undergoing a drug screen and/or alcohol testing at the beginning of employment, as well as after commencement of employment where there is reasonable suspicion of intoxication, or following an automobile or other accident, or as part of a company or facility-wide random drug screen process.

Fair Credit Reporting Act (FCRA):

Under the FCRA, Chloeta may request a credit check during the onboarding process. A written authorization form will be required before obtaining credit records.
