Job Summary

Performs intermediate work in public health planning. Conducts research on and assists in the planning, development, modification, and implementation of agency and/or division policy and strategic plans. Implements and evaluates programs and initiatives, with a focus on data analysis, health access and ensuring health equity. Assists in preparing reports and presentations. Works with clients, partners, outside vendors, and management to develop or implement programs, initiatives, and policy, systems, and environmental (PSE) changes in a manner that addresses agency and community strategies.

ESSENTIAL DUTIES AND RESPONSIBILITIES

Program Planning

  • Leads community health planning initiatives, including the Community Health Needs Assessment (CHNA) and Community Health Improvement Plan (CHIP). Recommends and supports development of data-informed strategies and programs in alignment with evidence-based practices and the CHIP or other community initiatives.
  • Assists in the development and implementation of the work plan to address goals, objectives, and strategies for improving health priorities identified by combining researched best-practices with community input.
  • Develops and implements health education materials to strengthen and support population health outcomes.
  • Develops and implements health equity strategies, policies, and procedures.

Accreditation

  • Coordinate public health accreditation process through Public Health Accreditation Board (PHAB). Facilitates and provides projects management for PHAB team. May serve as point of contact for PHAB accreditation organizing and maintain documentation for e-PHAB.

Needs & Asset Assessments

  • Assesses the health needs and resources of Chisago County residents. Conducts community health needs assessment (CHNA) and/or health impact assessments in conjunction with community partners across all sectors.
  • Contributes community health planning expertise to the community needs assessment process, health improvement goal setting, identification of public health policy and action options, program development, and creation of monitoring and evaluation plans.

Evaluations:

  • Performs evaluations of public health programs and planning processes. Evaluates effectiveness of community programs, interventions, initiatives, plans, and policies designed to improve population health, and uses this information to develop plans to carry out public health objectives.

Data

  • Ensures outcome measures are being collected and monitored.
  • Assists in efforts to collect, analyze, interpret, and disseminate accurate, reliable data and information about factors that impact population health.
  • Updates public health key performance indicators (KPIs) and business indicators at the county level. May analyze data, including statistical analysis.

Communications

  • Lead efforts to consistently and effectively inform, engage, listen to, assess, and motivate external and internal audiences.
  • Develop communication and marketing plans, such as news releases, brochures, consumer, and social media while ensuring accessibility of communications to varying audiences, especially those with disabilities.

Performance Management/Quality Improvement

  • Advances the County’s customer-centered culture principles, as well as the Health Department’s and the County’s mission, strategic plan, and core values.
  • Demonstrates a commitment to customer satisfaction for all customers.
  • Assists with performance improvement efforts, including aiding in the facilitation and development of performance goals and measures across the agency.
  • Designs and implements performance management/quality improvement processes, systems, communication strategies, and recognition activities.

Grant Writing

  • Writes applications for funding to support and enhance program needs. Participates in grants writing and submission efforts to increase funding for the work of the organization.
  • Provides support in data and equity in grant application.Under the direction of the CHS Administrator and/or program supervisors, supports development of grants.

Reports/Writing

  • Ensures monthly, quarterly, and annual program reports are submitted as required.
    • Specific Program Example: Statewide Health Improvement Plan (SHIP) Coordinator: Develops educational and promotional material, reports, plans, grant applications, and related communication tools to support health policies and programs.

    Research

  • Assists in the management of grants, once obtained, including supporting development of protocols, completion of progress reports. Assists with and conducts literature reviews and efforts to summarize findings that can inform the provision of evidenced based, high-impact prevention training and technical assistance as well as quality improvement activities. Produces tables,
  • Reports, presentations, and manuscripts based on findings, for public dissemination. Supports submission of abstracts to national conferences and papers to peer reviewed journals, as appropriate

Incident Command system role in Public Health Emergency Preparedness

  • Assists with updating preparedness plans and preparedness drills and participates in emergency preparedness activities with community partners as needed.

Performs other Duties as Assigned

  • Represents Chisago County Health Department at internal and external meetings, and events.
  • Employee must have demonstrated ability to work effectively and efficiently in a wide variety of computer programs and systems.
  • Must be able to maintain positive working relationship with County Attorney, CountyAdministration, Department Administrative team, and other county and communitycommittees, State offices of the State of Minnesota for miscellaneous programs.
  • Demonstrates adherence to and knowledge of HIPAA and Data Practices in workassignment.
  • Follow County and Department Policies and Procedures.
  • Always keeps and Maintains Automobile Insurance and valid Driver’s License.
  • Must be able to communicate in a variety of settings and have a demonstrated ability to convey at times complex information in public settings, both large and small.(Including development of presentations in various mediums.)
  • Assists Community Health Administrator and Health and Human Services Director as assigned.
  • May be expected to pursue or manage grants related to the work of the division in varying amounts.
  • May be given assignments requiring close work division accounting personnel.

Bachelor's degree in Public Health or related fields and moderate experience, or a combination of equivalent education and experience.

Special Qualifications

Valid driver’s license in the State of Minnesota.

  • Knowledge and experience in planning principles, methods and techniques including strategic planning and coalition building.
  • Qualitative and quantitative data collection and analysis.
  • Facilitation training and skills are highly desirable.
  • Competency and the ability to apply continuous quality improvement theory, and performance management in program evaluation.
  • Ability to diagram business processes.
  • Sufficient ability to understand the process of laws, rules, regulation and policy to assist in the development of public health policy and plans.
  • Ability to implement critical thinking analysis to public health surveillance systems for community health assessment work.
  • Experience and knowledge in project management tools and implementation.
  • Understanding of community engagement/public participation theories.
