Chino Hills Adult Day Health Care Center is currently looking for a social work assistant.

Qualifications:

• Fluent in English/Spanish or English/Mandarin
• BA/BS in Social work, sociology, gerontology, or psychology ONLY*.
• Experience working with the elderly.
• Social service experience a plus.

Duties (include but are not limited to):

• Conduct initial assessment and reassessment under the supervision of a MSW.
• Document flow sheet and progress notes.
• Case management (apply access, IHSS, etc.)
• Wellness Checks/Check in calls
• Other duties as assigned by Program Director

Job Type: Full-time

Pay: $20.00 - $23.00 per hour

Expected hours: 35 per week
