Job description
Horizons Care Direct Care Staff Job Description
Horizons Care provides a comprehensive array of services to include residential foster care, adoption, behavioral health therapy, targeted case management and crisis response to children, families and communities in Manatee, Sarasota, Desoto, Hillsborough and Pinellas counties.
Horizons Care provides effective, flexible and high-quality services in the Out of Home Care program, including 24- hour care in a highly structured residential setting for children and young people with high and complex needs, in accordance with their case and behavioral plan as well as direct care to foster care clients, foster caregivers, and their families. This role involves 24-hour care /7 days a week roster and requires staff to be available for a variety of shifts that include overnight stays.
Responsibilities
General:
Adhere to all current federal& state laws, as well as to Horizons Care Policies and Procedures, including the safeguarding of confidential healthcare information and compliance with the Health Insurance Portability Accountability Act (HIPPA).
Immediately reports any suspected incidents of child abuse or neglect to the Program Administrator and the Child Abuse Hotline.
· Reports serious incidents and updates to caseworkers.
· Attends agency meetings as required.
· Completes other duties as directed by the Program Administrator.
Duties:
Ensures the safety and basic care to the residents at Horizons Care.
1. Supervise and provide training to residents in personal hygiene, grooming, and dressing.
2. Have residents ready for school bus on time.
3. Prepare and supervise meals and snacks, providing training as per schedule.
4. Supervise and facilitate all recreational and leisure time.
5. Plan and facilitate activities (games, play, projects, including routine daily activities).
6. Keep house clean and laundry done.
7. Attends staff meetings and trainings as required.
8. Provide input in staff meetings to develop individual habilitation plans, as necessary.
9. Observe and note changes in behavior such as side effects of medication or symptoms of illness.
10. Follow individual habilitation and behavioral plans.
11. Document activities and maintain daily log.
12. Participate in 40 hours of in-service training annually to include management of aggressive behavior and psychotropic medications.
13. Transport residents in facility van to activities and appointments.
14. Administer and record medications.
15. If on duty at night, provide awake staff as needed.
16. Immediately report any suspected incident of child abuse, neglect or exploitation to the Child Abuse Hotline and to the Executive Director or Program Director and or staff supervisor.
17. Facilitate weekly house cleaning, teaching residents to see dirt and clean effectively.
18. Assess and correct any problems within skill level.
19. Submit work order request to Program Coordinator for any work that requires a skilled worker such as major electrical and plumbing issues.
20. Performs any other duties as assigned by the Program Administrator or Program Supervisor.

24. Documentation in the communication log DAILY ON EVERY SHIFT
25. Admissions of youth and room assignment
26. Assuring scheduling of 72-hour Wellness Admission Physical by Physician
27. Ensures the property, grounds, physical workplace areas are maintained to standard.
28. Performs sudden and unannounced audits on rooms and other operating areas of the facility.
29. Log all Report Cards and Progress Reports in youth file.
30. Attend School Conferences if needed and IEP Meetings.
31. School Enrollment.
32. Set up Transportation for Bus Ridder.

1. Ability to assist residents during meal preparation, housekeeping responsibilities, personal hygiene, grooming and other activities as needed.

7. Competent in using Microsoft Word, Excel, the Internet, and other software applications.
8. Effective oral/written communication and organizational skills
9. Ability to effectively work as a part of a professional team.

Salary: $15.00 - $19.00 per hour
