General Purpose of Position (Full job description will be provided upon interview)
To provide case management and support for teens identified as at risk of out of home
placement to preserve the family and keep the child in their home. This will include meeting
with parents and children in their homes or other settings to identify service needs and
overcome barriers to the family obtaining services. In some cases, this may require working
with the parent to get the child into a suitable treatment setting for mental health or
substance abuse. Participate in interagency planning meetings with DJJ, DCF and other
agencies to create plans to support the youth and family and then help to coordinate
services to meet their needs.
M I N I M U M R E Q U I R E M E N T S
Education / Experience / Certification
Bachelor’s Degree from an accredited college or university in social work, counseling,
human services, psychology, or related field, and at least 1 year of direct service
experience working with youth with youth and families. OR
Associate’s degree with 3 years of professional work experience with youth and families OR
High School diploma and 6 years of experience working with teens with difficult behaviors
and/or behavioral health or substance abuse disorders.
Requires experience working with teens with challenging behavioral issues.
Knowledge of local community resources and experience with linking families to services.
Experience working with youth in the child welfare and/or juvenile justice systems is preferred.
Certificates, License, Registrations, background checks, drug screens
Valid driver’s license required, a clean Children’s Network participates in E-Verify
driver’s license check (required), and proof Criminal background screening mandatory
of insurance (local law enforcement / FBI / FDLE, and Abuse
Drug Screening is required Registry)
