The Outreach Liaison is responsible for participating in a comprehensive, statewide outreach program. Focuses on building positive relationships and business with all facets of the market including primary care providers, subspecialty care providers, and other healthcare organizations.

Interacts with patients families, other hospital staff and referring physicians and facilities in a professional manner.

Job Description

This job description is intended to describe the general nature and level of work performed by employees assigned to this department.

GENERAL DUTIES

  • Assists in the delivery of a comprehensive primary care outreach model to engage community referral sources, problem solve for community practices and to overall increase the use of and preference for organization's services.
  • Visits physician groups in accordance with the established schedule to increase awareness of the specialists and services of the facility.
  • Develops and implements plans to establish solid referral lines. :
  • Assists with concierge scheduling to improve access to care.
  • Coordinates physician and hospital outreach referral visits. Follow-up with referring physicians or hospitals to ensure satisfactory service and growth in business.
  • Develops and maintains positive working relationships with physicians and their staff, and work proactively to resolve physician concerns to promote future growth and development.
  • Helps with data collection and reporting related to outreach :
  • Completes ongoing reports on all liaison and educational activities. Document concierge scheduling requests and related metrics.
  • Submits monthly expense reports covering in a timely manner to assist with business planning.
  • Represents the department in community events and provide appropriate follow-up and resolution to any issues that improves customer relationships.

EXPERIENCE QUALIFICATIONS

2 years / 3-5 years Physician practice or equivalent capacities

EDUCATION QUALIFICATIONS

  • Required : Bachelor's Degree Business, Nursing, related field
  • Preferred : Master's Degree Business, Nursing, related field

SKILLS AND ABILITIES

  • Ability to communicate clearly and concisely in person and on the phone along with strong interpersonal skills required.
  • A positive, empathetic, and non-judgmental style of interaction with patients, physicians, co-workers, and the public.
  • Ability to work independently and prioritize tasks without direct supervision.
  • Organizational skills with an attention to detail.
  • Knowledge of physician outreach and referring providers’ / organization’s concerns

FUNCTIONAL DEMANDS

  • Light : Light physical requirements- Light Work - Exerting up to pounds of force occasionally, and / or up to pounds of force frequently, and / or a negligible amount of force constantly (Constantly : activity or condition exists 2 / 3 or more of the time) to move objects.
  • Physical demand requirements are in excess of those for Sedentary Work. Even though the weight lifted may be only a negligible amount, a job should be rated Light Work : (1) when it requires walking or standing to a significant degree;

or (2) when it requires sitting most of the time but entails pushing and / or pulling of arm or leg controls; and / or (3) when the job requires working at a production rate pace entailing the constant pushing and / or pulling of materials even though the weight of those materials is negligible.

NOTE : The constant stress and strain of maintaining a production rate pace, especially in an industrial setting, can be and is physically demanding of a worker even though the amount of force exerted is negligible.
