Job OverviewThe CHIP Clinic serves children, youth, young adults, pregnant women and families living with, affected by, or at-risk for Human Immunodeficiency Virus (HIV) with support from federal, state, local and research grants. The Health Navigator role provides case management support in concert with the psychosocial and medical team for CHIP programs, patients, and their family members. Health Navigator navigates internal and external databases for compliance of client eligibility requirements to the Ryan White Care Act. Role requires ability to build authentic relationships with diverse communities including communities of color, LGBTQ community, and socio-economically diverse communities. Excellent written and verbal communication skills, and ability to actively engage with community of HIV service providers. Qualifications
  • EDUCATION – High School Diploma or equivalent.
  • EXPERIENCE – One to three years of experience assisting patients and families or other related healthcare/community experience. Experience working directly with people from diverse communities. Experience with case management and administrative duties.
  • EQUIVALENCY – An Associate degree, in a related field, may substitute for the experience requirements on a year for year basis.
  • CERTIFICATION(S) – Community Health Worker Certificate, Patient Navigator Training Certificate, and/or Ryan White Medical Case Management Certificate preferred.
ResponsibilitiesPOPULATION SPECIFIC CARE
  • No direct patient care.
Essential FunctionsProvides Linkage to Care for clients new to HIV care at CHIP. Serves as first point of contact and requires ability to authentically build rapport with clients, provide support, and address barriers to engagement for diverse communities including communities of color, LGBTQ community, and socio-economically diverse communities. Provides on-call (during business hours) coordination and support for confirmatory HIV testing.
Maintains CHIP’s process for verifying Ryan White resource eligibility in accordance Ryan White Standards of Care including obtaining eligibility verification documents for all patients twice a year, maintain excel workbook of monthly eligibility review and represents this effort at program monitoring visits.
Provides insurance navigation and enrollment for patients living with HIV. Evaluating client eligibility and enrollment in the State Drug Assistance Programs and health insurance options based on income, residency, and HIV status, in accordance with the HIV Care and Treatment Program Standards of Care
Plans and coordinates psychosocial events, groups and classes for CHIP patients and their families in concert with CHIP Clinical Social Workers
Provides care navigation and case management services for Pre (HIV) Exposure Prophylaxis “PrEP” patients entering care at CHIP or provides community referrals.
Provides Retention in Care services including organizing team discussion, documents retention actions steps and follows operating procedure for Maintaining Engagement in Care standards.

Excellent written and verbal communication skills, and ability to present to diverse audiences.

Salary Information
Annual Salary Range (Based on 40 hours worked per week): $50,911.60 to $76,367.40
Hourly Salary Range: $24.48 to $36.72
