As a Supervisor Case Manager, you will be responsible for leading and supervising a team of case managers in the effective implementation of programs and services for clients. Your main goal will be to ensure the quality of services provided, operational efficiency, and compliance with established standards. Additionally, you will provide support and guidance to the team, fostering a collaborative and professional development environment.

Responsibilities:

Supervise and guide a team of case managers in the execution of their daily tasks.

Provide ongoing guidance and training to the team, ensuring their growth and professional development.

Monitor team performance and set clear goals and objectives.

Conduct regular performance reviews and provide constructive feedback to team members.

Coordinate and assign cases, ensuring a fair and efficient workload distribution.

Participate in the selection and hiring of new members for the case management team.

Collaborate with other departments and healthcare professionals to ensure comprehensive and coordinated care for clients.

Supervise compliance with organizational standards and policies.

Monitor case reports and records, ensuring accuracy and confidentiality of information.

Identify areas for improvement in case management processes and procedures, proposing efficient solutions.

Act as a liaison between the case management team and organizational management, effectively communicating team challenges, achievements, and needs.

Requirements:

Bachelor's degree in Social Work, Psychology, Nursing, or a related field. (Additional licensure or certification in case management will be valued).

Previous experience in case management or a similar supervisory role.

Strong knowledge of case management principles and practices.

Demonstrated leadership skills, with the ability to motivate and guide a team.

Excellent oral and written communication skills.

Ability to work in a dynamic and high-pressure environment.

Knowledge of community resources and services available to clients.

Ability to make effective decisions and problem-solve.

Strong work ethic and commitment to confidentiality.

Proficiency in computer tools and case management systems.
