Job Description Performs patient and family telephonic outreach to gather data regarding care patient receives in community as compared to treatment plan. Documents result of outreach calls using standard tools and workflows. Assesses, organizes, synthesizes, and graphically displays data collected for surveillance. Ensures that data produced is valid, meaningful, and reliable. Proactively supports the efforts of the Center for Behavioral and Mental Health at Children’s Healthcare of Atlanta.
Experience
6 months of patient or customer-facing interaction, in person or telephonic

Master’s degree
General knowledge of healthcare workflows
Education
Bachelor’s degree

Articulate, warm, and professional when speaking to patients on the phone
Able to efficiently use time to execute and document patient outreach
Competency with spreadsheets, word processing, and database software
Ability to research, manipulate, and analyze data and identify implications
Ability to display data in graphical formats, including aggregation and analysis summaries
Ability to use internal and external information systems
Ability to communicate well and present findings verbally and in writing
Ability to support multiple assignments, shift priorities in accordance with need, and maintain confidentiality of sensitive information
Good interpersonal, communication, and conflict resolution skills
Ability to take direction from others who are not a direct supervisor
Job Responsibilities
Maintain extensive records and large data sets pertaining to outcomes of patient outreach calls.
Interact with other hospital staff and/or departments and external organizations and request/collect data as it applies to the stated requirements.
Collaborate with supervisors to prepare data for BMH leadership to support decision-making.
Perform other department-specific responsibilities related to database maintenance and/or data collection.
Ensure the accurate, timely entry of all collected clinical and operational data.
Adhere to data definitions, as directed by supervisor.
Adhere to Health Insurance Portability and Accountability Act requirements pertaining to strict confidentiality regarding patient data, quality assurance indicators, and results of information collected.
