Children Home is Hiring a Registered Nurse (RN) - Assertive Community Treatment (ACT) Near Chenango County, NY

Job Summary:

The Children's Home is seeking a dedicated Registered Nurse (RN) to join our Assertive Community Treatment (ACT) team Chenango County. The purpose of ACT is to meet the needs of individuals who have not been well served by traditional service delivery approaches. Grounded in a recovery-oriented practice, ACT offers evidence-based treatment, rehabilitation, case management, and support services through a mobile, multidisciplinary behavioral health treatment team.

The RN will play a crucial role in conducting assessments, addressing basic health and medical needs, and coordinating healthcare with community providers. Additionally, the RN will collaborate with the psychiatrist/Nurse Practitioner (NPP) in medication management and administration, while also delivering a variety of treatment, rehabilitation, education, and support services.

Responsibilities:

· Conduct assessments to evaluate the health status and needs of individuals.

· Address basic health and medical needs of clients under the supervision of the medical team.

· Coordinate healthcare services with community medical providers, including primary care physicians and specialists.

· Assist in the management and administration of medications under the direction of the psychiatrist/NPP.

· Provide a range of treatment, rehabilitation, education, and support services to clients.

· Collaborate with the multidisciplinary team to develop and implement individualized care plans.

Qualifications:

· Registered Nurse (RN) license in the state of New York.

· Experience in conducting health assessments and coordinating healthcare services.

· Knowledge of medication management and administration practices.

· Strong communication and interpersonal skills.

· Ability to work collaboratively in a multidisciplinary team environment.

· Commitment to providing high-quality care and support to individuals with mental health needs.
