Children Home is Hiring a Family Engagement Coordinator Near Binghamton, NY

The Children's Home is looking for a dependable Family Engagement Coordinator for our Residential Program!!

Job Summary: The Family Engagement Coordinator is a critical member of the residential hard to place dorms. The Family Engagement Coordinator handles many details of the cases and frees up the social worker to focus on therapy and groups. The Family Engagement Coordinator is responsible for developing the after care plan starting with the youth’s placement on the dorm.

Job Responsibilities:

  • Co-facilitate group counseling with Social Worker.
  • Gather information for the Social Worker to create the Treatment plan.
  • Gather information from families for the Individualized Crisis Management Plan (ICMP), 30 psychosocial, etc.
  • Will need to be trained in Child Adolescent Needs and Strengths Assessment.
  • Schedule home visits with families, write visitation plan, schedule transportation, and complete before and after notes in Connections.
  • Maintain communication with family member(s) for each youth.
  • Provide casework services related to discharge planning and the youth's re-entry into the family and community.
  • Support the Social Worker in identifying permanency barriers, kinship resources, and visiting/mentoring opportunities early in placement

*Perform any and all miscellaneous job duties as may be required from time to time.*

Qualifications:

  • Bachelor's Degree in social work, human services or related field from an accredited college or university AND 3 years' relevant experience preferred.
  • Minimum Associate's Degree in social work, human services or related field from an accredited college or university AND 2 years' related experience required.
  • Must have valid driver's license and meet minimum insurability requirements during employment.

Relevant Knowledge & Essential Skills: The ideal candidate will demonstrate the ability to work effectively with clients, families, staff, and community contacts with a diversity of backgrounds and life situations. The Family Engagement Coordinator must be able to participate in the principles and practices of the inter-disciplinary team approach. The Family Engagement Coordinator must possess an understanding of and sensitivity to serving culturally diverse and special needs population. Excellent verbal and written communication, problem solving and organizational skills required. Must be able to write reports/case notes. Must be able to use laptop/desktop computers for consistent documentation into assigned youth electronic heath record (EHR) charts.

Job Type: Full-time

Pay: $21.00 - $23.00 per hour
