Job Responsibilities
Conducting intakes and screenings and ensuring all clinical forms are completed and signed
Coordinating the process of evaluations and assessments to determine initial and on-going eligibility
Measuring and tracking progress and outcomes for young people and families
Attending and participating in weekly Team Meetings, agency staff meetings and Trainings
Coordinating services with community resources, medical and health providers
Assisting with crisis-management and supervision when necessary
Communicating effectively with families, Child and Family Team members (stakeholders therapists, case managers, etc.), agency management and Direct Support Specialist Coordinators
Performing other duties related to the position
Adaptability in scheduling, driving throughout the county and working within the homes and communities of the families we support is required.
Education / Experience Needed :
Related Master's degree and prior behavioral health experience preferred
Requirements :
Must be 21 years or older
Valid Arizona Driver's License with an acceptable 39 month driving record
Reliable transportation, including Proof of Insurance
Candidates are required to submit a negative drug test
Obtain an Arizona DPS Level One Fingerprint Clearance Card
Complete Adult, Child, and Infant CPR and First Aid training
