ResponsibilitiesPerform patient comprehensive assessments, collaborating seamlessly with our multi-disciplinary team of experts, including RN, PT, OT, ST, MSW, and paraprofessionals. Together, we will work passionately to achieve the heartfelt outcomes and goals of our patients, leading them on a journey of healing and hope.
Lead with Heart - Nurturing Care Coordination
Work your caseload, nurturing the growth and development of Home Health Aides, as we cultivate a harmonious team of caregivers.
With us, your diligent documentation paints a vivid picture of our patients' journeys towards wellness. With the ease of a tablet, you capture the essence of OASIS assessments, SOC/Admissions, recertification, physician orders, and more - weaving together a tapestry of care that surpasses expectations.
Qualifications- Active LPN License
- One (1 ) Year of Clinical Experience
- CPR Certification
- Valid Driver's License
