Bachelor’s degree required in Human Services, Education or Health Care and two years of experience in case management, human services or public health field or an equivalent combination of training and experience is required. Knowledge of infant/early childhood growth and development, basic teaching/learning principles and practices; and the concepts of positive parent/child relationships needed. Skills in organization and documentation; PC skills required. Ability to identify services and resources within the community including natural supports. Must be able to work independently, communicate effectively orally and in writing and work well with culturally diverse families. Current valid driver’s license and good driving record required. Based on the Virginia DMV point system, record must not reflect a total of six or more demerit points within the twenty-four months preceding the anticipated hire date, or one major violation of six demerit points within the preceding thirty-six months. Out of state records must be obtained by applicants and presented at time of interview. Records must reflect at least three years of history and be dated within thirty days of interview date. Pre-employment drug testing, FBI criminal background check and education/degree verification required. Provide targeted case management, education, screenings, and support to at risk families of young children through bi-monthly home visits. Assist families directly to obtain needed resources and services. Provide developmentally age-appropriate activities and materials to families using the Parents As Teachers home visiting model. Perform other work as required.
