Working in an integrated team model, the RN’s role will include working directly with and in a support role of a panel of patients.

• RN License in the State of Michigan
• Two years working with a diverse patient population
• Bilingual in Spanish and English preferred

• Complete patient assessment, education and, as appropriate, patient scheduling telephonically and in person through walk-in appointments
• Utilize knowledge in conjunction with approved protocols and standards in advising patients and parents
• Conduct one-on-one or family patient education sessions
• Assist in coordinating services for patients and families utilizing other health-related agencies and resources
• Provide follow-up for patients on health care concerns
• Participate in interdisciplinary planning for meeting patient needs and health service objectives
• Participate in the quality assurance program
• Perform other duties as assigned
