About our Community Health Workers:

Our Community Health Workers are responsible for establishing and maintaining relationships with the community, care teams, and patients. Their main goal is to educate and link patients to care. By supporting our patients, providers, and community, our Community Health Workers engage in valuable clinical, research, and advocacy initiatives to increase our patient's awareness of available health prevention services and research opportunities at Cherokee Health Systems.

What we are looking for:

A compassionate advocate that has a high school diploma with 1 to 2 years' experience with linking patients with various community resources.

What you will do:

  • Collaborate with clinical and administrative leadership to support education, community outreach, and program development to enhance access to care, with specific focus on marginalized populations
  • Provide advocacy, linkage, and referral to CHS and community resources.
  • Develop working relationships with community resources including those providing services to target populations.
  • Collaborate with existing community partners to maintain relationships, enhance access to care, linkage to care, and address gaps in care for potential patients.
  • Promote health education and encourage patients to establish a healthcare home.
  • Attending community events to disseminate educational material on health conditions including but not limited to HIV/AIDS, Hep C, substance misuse, as well as efforts at prevention including PrEP, PEP, and risk reduction strategies.
  • Coordinate communication and engagement efforts for CHS research programs as designated by Director of Research and Health Equity
  • Collaborate with CHS’ Director of Development to coordinate and manage CHS social media accounts to promote resources and programs offered at CHS and within community
  • Educate and encourage community members and potential patients to engage in preventative care.
  • Educate community members about the benefits of health research and encourage participation
  • Complete reporting requirements as required/requested.
  • Maintain professional relationships with patients and with other staff and maintain appropriate interpersonal boundaries.
  • Other duties as assigned.
