JOB TITLE: SOCIAL SERVICES ADVOCATE
DEPARTMENT: ICWA
POSITION TYPE: FULL TIME
SALARY: $ 17.00-18.00/hr:
ROLE AND RESPONSIBILITIES::
  • Conducts comprehensive assessments and use intake forms to determine appropriate housing options, case management and other supportive service needs.
  • Develops individualized case plans with measurable goals and objectives to assist participants in achieving their desired outcomes.
  • Conducts on-going quarterly needs and financial assessments to identify client’s continued housing plan, supportive service needs and barriers to securing/maintaining permanent supportive housing.
  • Works with program participants to develop a housing stability plan and budget (this includes a service plan)
  • Assists participants to get “document ready” by supporting them to obtain IDs, Social Security cards, birth certificates, and other key documents needed to successfully obtain housing and link to appropriate supportive services, including letters of reference for landlords.
  • Assists participants with securing and/or increasing income (once housed) including connections to mainstream benefits.
  • Networks and coordinates with representatives from other agencies & community-based organizations to support individuals in attaining services such as housing, mental health care, medical treatment, financial assistance, legal advocacy, etc.
  • Identifies long-term affordable housing resources (Such as Chemehuevi Housing Department, Tribally managed housing units, etc.) and assists clients to pursue options with referrals and support filling out applications.
  • Coordinates housing search activities with the Housing Locator and attends unit/apartment viewings with participants.
  • Educates participants on tenant rights, responsibilities, and how to be a good tenant.
  • Provides tenant support when housing-related issues occur.
  • Assists participants with transitioning into permanent housing and provides supportive services to improve participants overall housing stability.
  • Performs home visits, eviction prevention, budgeting, life skills coaching, and other housing retention services or referrals to support clients with sustaining permanent supportive housing.
  • Prepares, maintains, and updates clients’ hard case files/e-files related to the case for review and record keeping purposes. Ensures timely completion of client case notes.
  • Models ICWA Department’s approach, mission, and core values (including Housing First, Harm Reduction & Family Preservation) in all communication, correspondence, community events, coalitions and advocacy efforts.
  • Reports to work on time and maintains reliable work performance.
  • Prepares purchase orders/check requests, travel/training Requests, data entry reports (as required), maintains tracking logs.
    • Other duties as assigned.
    QUALIFICATIONS::
    Requirements/Qualifications: :
    • Experience in the human services, social work, medical, or related field.
    • Ability to gather complete information from clients in a telephone interview or in person interview.
    • Ability to respond to clients in a calm, empathetic and professional manner while courteously directing the conversation to pertinent issues.
    • Familiarity with the principles of harm reduction and housing first.
    • Excellent written and oral communication skills.
    • Ability to work independently as well as support team efforts, and to strategize and plan so as to timely and consistently support clients to find and maintain housing.
    • Excellent organizational skills to handle individual caseload of housing case management clients.
    • Microsoft Office skills.
    Desired Knowledge, Skills and Abilities: :
    • Experience as a Social Services Advocate
    • Experience working with individuals who are homeless or at risk of being homeless (e.g., individuals, families, children, Veterans, etc.) who have complex health and/or behavioral health conditions and/or at risk for Adult Protective Service/Child Welfare Service intervention.
    • Strong client customer service skills, including working from a trauma-informed perspective with clients.
    • Demonstrated commitment to serving Chemehuevi Families experiencing or at-risk of homelessness.
