CHC Addiction Services is Hiring a Client Engagement Specialist Near Akron, OH
We are currently looking for a Client Engagement Specialist for our Outpatient program. The Client Engagement Specialist directs patients through substance abuse treatment through interactions with both CHC Addiction Services and in the community.
- Provide community outreach and information about behavioral health and addiction treatment services and access to care to individuals, groups, and other potential referral sources
- Facilitate client engagement in treatment by assisting with barrier removal and coordination of care with referral sources and external entities
- Completing appropriate case recording requirements
- Communicate with patient treatment providers
- Provide patient access to individualized treatment
- Promote patient participation in outreach activities
- Present patient cases weekly in clinical supervision meetings and seek additional professional consultation as needed
