Job Description

POSITION SUMMARY

The goal of the Health Navigator is to help enhance access to high-quality medical services in order to improve the overall health status and lower the cost of care for OHS eligible families. As part of the OHS team, the Navigator will focus on performance measure dashboards to help identify and close gaps in care. The Navigator will accomplish this goal by identifying patients eligible for OHS services and patients in need of healthcare services supporting efforts to ensure completion of those services. The Navigator will work with the OHS Program Manager and other members of the OHS team to review current approaches and develop data-informed improvement strategies The Navigator works closely with CDHC clinical team members, specifically those providing Obstetric and Pediatric services. The Navigator will also provide education related to health services to CDHC patients in an effort to raise awareness and improve access to services.
POSITION-SPECIFIC DUTIES & RESPONSIBILITIES

  • Understands and is adept in using maternal child health tools within CDHC EHR as well as data in Well Family System.
  • Reviews reports generated from maternal child health applications (including EHR) to identify patients due for specific medical services and screenings.
  • Contacts patients due for specific medical services and screenings and assists them with making necessary appointments.
  • Coordinates with OHS and clinical team to assist patients in the completion of screening services by providing follow-up and support.
  • Obtains clinical data from external organizations, enters data into EHR, distributes data to providers and OHS team as well as patients, when necessary.
  • Acts as a patient liaison by ensuring patients connect with the Eligibility and Enrollment team, assisting patients in making appointments for specialty services, and providing educational support.
  • Collaborates with outside agencies to obtain specialty services for our patients where necessary.

POSITION REQUIREMENTS

  • Education: High School Diploma or equivalency required. Bachelors in Human Services field Preferred but not required.
  • Experience: Patient/client navigation services preferred.
  • Expertise: Strong interpersonal skills.
  • Proficient verbal, written and reporting skills.
  • Microsoft applications (experience with Excel required), internet, email Good record keeping methods.
  • Ability to achieve measureable outcomes Strong organizational skills.
  • Ability to handle multiple tasks Knowledge of medical terminology.
  • Language: English fluency (Karen Preferred).
  • Work Hours: Monday –Friday 8:00am – 5:00pm; flexibility to adjust to meet patients during clinic operation hours.
  • Travel: Minimal

In all cases personal protective equipment will be provided to the employee in combination with adequate ventilation and other engineering controls to minimize the risk of exposure or other hazardous occurrence.
