POSITION SUMMARY:

The Diabetes Program Assistant position is responsible for assisting the Diabetes Program Manager (DPM) and Diabetes Department employees with DPP and Diabetes classes, retinal screens and other administrative duties as needed. Must be able to travel to both Grass Valley and Auburn locations.

DUTIES AND RESPONSIBILITIES:

  • Responsible for ensuring retinal screenings are performed annually for clients with diabetes by identifying, contacting and scheduling a retinal screen.
  • Responsible for administrative assistance duties for the Diabetes Educators, Diabetes Program Manager for team meetings, Diabetes Prevention Program classes and support groups.
  • Assist Diabetes Team in keeping statistics for DPP recognition.
  • Routinely monitor referrals through eCW to the Diabetes department and schedule appointments with the appropriate team member.
  • Schedule follow up appointments for Diabetes Team members, PCP and Ophthalmology as requested.


Required Experience:


QUALIFICATIONS:

  • Minimum of 6 month experience as a Program Assistant in a health related field.
  • Experience in food preparation and cooking with food safety certification.
  • Understanding of basic computer literacy and word processing systems.
  • Ability to work independently and as part of a team.
  • Excellent oral and written communication skills.
  • Experience working with the American Indian community (preferred).
