We're seeking an RN to join our healthcare team. The perfect candidate will possess expertise in home health, wound care, geriatrics, and outpatient services. Responsibilities include coordinating patient care plans, scheduling appointments, and offering support to patients and families. This role calls for a compassionate individual with stellar communication skills and the capacity to engage effectively with diverse patient populations.

Responsibilities:

• Provide skilled nursing care to patients in their homes.
• Perform various medical procedures such as gastric lavage, spinal tap, and airway management.
• Administer medications and treatments as prescribed by the physician.
• Monitor and assess patient's condition, including vital signs and symptoms.
• Collaborate with other healthcare professionals to develop and implement a comprehensive care plan
• Educate patients and their families on disease management, health coaching, and more.
• Ensure infection control measures are followed at all times.
• Coordinate discharge planning and provide necessary support to patients transitioning from hospital to home.

Qualifications:

• Valid Registered Nurse (RN) license in the state of CA.
• Minimum of 1 year of nursing experience, preferably in home health or a related field.
• Strong clinical skills and knowledge of various medical procedures.
• Excellent communication and interpersonal skills.
• Ability to work independently and manage time effectively.
• Compassionate and patient-centered approach to care.
• Fluent Spanish speaker.

Job Types: Part-time, Per diem

Pay: $150.00 - $170.00 per hour

Expected hours: 5 – 20 per week
