We are looking to add a Social Services Designee to our team!

Job responsibilities will include but not limited to:

Initiating and completing care conferences on new admissions, as well as existing residents according to state regulations.

Coordinating resident discharges, setting up in home therapy, DME equipment, etc.

Being available to residents and their families for grievances/questions.

This social worker designee would be responsible for both of our campuses, Cedar Hill and Sterling Suites. With it being a high-volume position, prior experience is required.

We are looking for an individual with experience in:

• Assisting clients with planning and finding community services necessary for success
• Interactions with clients and community agencies
• Handling and completing paperwork
• Working with clients who have different levels of physical, cognitive, or psychological limitations

This position is ideal for someone who is looking to enter the social services field and has minimal prior experience. We are looking for this person to assist with planning and finding community services necessary for the success of residents within our community and overall create a positive environment.
