The Occupational Therapist is responsible and accountable for residents’ evaluation, ongoing assessment, planning, and implementation of therapy interventions.
Responsibilities:Develops and revises plan of care based on the residents’ needs and establishes functional goals, outcomes, and discharge plan. Plans for residents’ discharge and anticipates for long term and/or short term residents’ needs. Educates other health care team members, residents, family, and caregivers on information regarding the care plan for the residents.
