Position Summary
The Assistant Director of Nursing will supervise the day-to-day operations of the Nursing Department in accordance with current regulations to ensure the highest degree of quality of care is maintained at all times. Highlands Rehabilitation and Healthcare Center is dedicated to providing the best care possible and that’s where you come in!
Experience:
Minimum of two (2) years clinical experience in a supervisory role in a long term care facility is preferred.
Valid Registered Nurse (RN) License
Knowledge of long-term care regulations preferred
Duties and Responsibilities
Assist the Director of Nursing Services in planning, developing, organizing, implementing, evaluating, and directing the day-to-day functions of the Nursing Department, in accordance with current rules, regulations, and guidelines that govern the long term care facility.
Participate in developing, maintaining, and updating our written policies and procedures that govern the day-to-day functions of the Nursing Department.
Ensure that all Nursing personnel are following their respective job descriptions. Participate in developing, maintaining, and periodically updating the written job descriptions for each level of Nursing personnel in accordance with pertinent laws and regulations.
Participate in facility surveys (inspections) from regulatory agencies as requested by the Administrator or Director of Nursing.
Review all new admission charts to ensure all assessments are complete and correct; all medications and treatments are correct per physician orders including appropriate diagnoses; the resident care plan is initiated; the Nursing Assistant documentation is initiated.
Create mandating schedule for nurses and nursing assistants.
Review nursing assignment sheets for each nursing unit; ensure nursing staff are following assignments.
Maintain the confidentiality of all resident care information, and ensure the respect of all residents’ privacy.
Maintain the RN Report Sheet; ensure all information on the report is correct.
Complete reports and audits timely: 802/672 (matrix and census and condition); Infection Control Log; Oxygen Log; Psychotropic Medication Log, Hospital Transfer Log, etc.
Ensure all resident Event Reports are completed timely and signed by the Medical Director and log if necessary.
Ensure quarterly assessments/recaps and other Charge Nurse assignments are completed timely.
Review resident Hydration Book/Weight Book; ensure areas are complete and the physicians are notified when necessary.
Review MARs/TARs and CNA books monthly for errors, blanks, etc. and address any discrepancies.
Update Nursing schedules as necessary.
Assistant in educating/disciplining Nursing personnel when necessary.
Review Narcotic Shift to Shift books and Narcotic Reconciliation sheets for blanks and address as necessary.
Prepare disciplinary notices for Nursing Department when necessary.
Be able to organize work and prioritize.
Attend necessary meetings, including mandatory in-services.
Practice safe work habits.
Perform LPN Charge Nurse or CNA functions as needed or directed.
Problem solve all Nursing issues/concerns with Supervisors, Charge Nurses, Nursing Assistants. Notify the Director of Nursing of the concern and what you did to correct it
Accountable for 24 hours of nursing care for a specific unit. Delegate tasks professionally and follow through with what was delegated.
Maintain a professional manner with all residents, families, and coworkers.
Ensure resident call bells are answered timely and Call Bell Audits are completed and submitted daily.
Environmental Daily Rounds: resident appearance, infection control standards being followed, MARs not left open when unattended, med rooms are clean and organized, nurses’ station clean and organized, audits completed timely, Nurse Aide documentation completed correctly, all consults and recommendations are read and followed up with, labs are reviewed and on the chart.
Dining Experience: all residents are positioned properly for meals, staff sitting to assist residents with meals, staff conversing with residents and not other staff, residents at the same table are served at the same time.
Attend weekly Medicare Meetings, Care Plan Meetings, etc.
Prepare all information needed to report to Corporate for any unplanned hospital transfers for Medicare residents and DOH reportables: face sheet, diagnosis sheet, physician orders, nurses’ notes-past week, recent lab work, care plan.
