Position Title: Program Supervisor

Department: Family Strengthening

Salary: $25.00-$27.69 per hour

FTE Classification: 37.5 hours/week, FTE Non-Exempt

Report To: Program Manager

Location: CLFA Neighborhood Resource Centers

Job Announcement: Centro La Familia Advocacy Services (CLFA) is seeking a qualified candidate to implement services for its CalWORKs Hove Visiting Program (HVP). HVP is designed to support positive health, development and well-being outcomes for pregnant and parenting women, families and infants born into poverty. Using an evidence-based curriculum, staff will carry out services in a culturally responsive manner and oversee a team of Home Visitors who provide guidance, coaching, access to prenatal and postnatal care, and other health and social services. The candidate will ensure that services are delivered in a whole-family approach manner and report to the Program Manager. This position is funded by the Fresno County Department of Social Services.

Position Summary: The Program Supervisor will coordinate and oversee implementation of Home Visitation Program services in CLFA’s Neighborhood Resource Centers (NRCs) located in 93706, 93722, and 93630 (Kerman). The program uses the Parents as Teachers (PAT) model and curriculum and provides home visits to targeted parents enrolled in Fresno County Department of Social Services CalWORKs program. Services will address reducing child maltreatment, improving parent-child interactions and children's social-emotional well-being, and promoting children’s school readiness. The Program Supervisor will be responsible for providing administrative and reflective support and supervision to Home Visitors, conduct case management and data entry duties, assist with reports and data tracking, and other program needs while ensuring deliverables are on target. The candidate will also provide support to linkage services, outreach education at local events, and presentations. The Program Supervisor repots to the HVP Program Manager.

Essential Job Responsibilities

  • Responsible for assisting with the administration, planning, organization, and operation of the program
  • Implementation and adherence to the PAT model and curriculum
  • Assist with the supervision of Home Visitors and create benchmarks that are attainable and sustainable
  • Become knowledgeable about CLFA’s Case Management services and ensure they are followed by Home Visitation Staff
  • Responsible for coordinating with Home Visitors to designated area(s), as well as monitor their time, attendance, and schedule
  • Track all activities in the program using established tracking tools and case management activities
  • Support in recruiting, onboarding, and the training and development of staff
  • Lead, inspire, cultivate, and manage staff
  • Ensure that client caseloads are being tracked and maintained by staff
  • Ensure Home Visitors are following CLFA’s case management processes for all delivery of services
  • Follow all confidentially processes to protect client’s information
  • Provide community education by attending outreach events resource fairs, forums, presentations, and others
  • Track community education provided including sign in sheets, materials distributed, others
  • Participate in weekly and monthly meetings in person or virtually
  • Participate in identified trainings including but not limited to home visitation, confidentiality, and others in person or virtually
  • Responsible for reports: meet weekly and monthly reporting requirements including the Monthly Activity Report and others
  • Ensure all requirements and deliverables are met
  • Maintain calendar via outlook tracking appointments, events, meetings, trainings, and others
  • Assist with special projects/duties as assigned
  • Ability to work some evenings and weekends
  • Bachelor’s or Master’s degree in public health, health science, business administration, or related field
  • Experience working full time as a Program Manager, Project Coordinator and supervision of staff
  • Understanding of the Promotora model
  • Excellent interpersonal skills to assist distressed individuals from different cultural backgrounds
  • Experience working in community outreach, and/or education programs preferred
  • Bilingual/Bicultural
  • Access to a telephone, computer, and reliable internet connection
  • Proficiency in MS Word, Excel, and Outlook Express, as well as data entry
  • Ability to comply with regulations and communicate official information in an appropriate manner
  • Advanced ability to coordinate efforts, facilitate intervention, and maintain records.
  • Exceptional organizational skills and sound judgment
  • Valid CA driver’s license and auto insurance

Requirements/Working Conditions:

· Maintain a flexible schedule to achieve program deliverables required

· Develop a sense of belonging and teamwork among CLFA staff and collaborative partners to achieve desired outcomes

Reporting & Record-Keeping:

· All work-related information is stored and maintained on internal servers

· Utilize data collection tools to document program activities for reporting

· Keep all files/matters accurate and confidential pursuant to CLFA’s established case file protocols and policies

· Document the timely completion of contractual goals and objectives, including internal and external reports

· Use tools to document and track program progress which include: demographic forms, screening tools, assessment outcomes, updates on social media, sign in sheets, etc.

TRAINING AND PROFESSIONAL DEVELOPMENT

· Satisfactorily completion of assigned trainings and coordination efforts with subcontractor, evaluator and partners as required

· Attend mandatory training as part of their orientation and onboarding and will comply with state and federal regulatory requirements related to protected health information and privacy

TYPICAL WORKING CONDITIONS: The office setting is a normal environment. Occasionally work during early morning, evening, or weekend. May me subject to temperature variances outdoor during events.

TYPICAL PHYSICAL DEMANDS: Requires sitting, standing, or walking for up to eight hours a day. Some bending, stretching, or reaching may be necessary. Lifting to 40 pounds may be required on occasion. Vision must be correctable to 20/20 and hearing must be in the normal range for telephone contact.
