Position Description

The Maternal Health Coordinator is responsible for managing Centro’s ‘’Cuna’’ pilot program in collaboration with Cherokee Health Systems, revolving around maternal prenatal education and postpartum support. This position will lead the execution and evaluation of the program, being responsible for two successful cohorts. The position will require skills in health literacy, group facilitation and case management. The ideal candidate would have a background in health, managing programs, working with immigrant communities, and/or group facilitation. This is a one year grant based position to lead a group of expecting mothers, facilitating culturally responsive and trauma informed prenatal education as well postpartum support. Candidate must be a team player, reliable and enthusiastic.

Responsibilities

  • Bridge communication between Centro Hispano and Cherokee Health Systems
  • Facilitate community and support for cohort of expectant mothers through programming, communication, and
  • Coordinate all activities for the CUNA program including but not limited to: communication with program participants and partners, scheduling guest speakers, childcare, materials, medical interpreters, etc.
  • Coordinate with Centro’s medical interpreter program coordinator for best practices when working with expectant mothers and workflow of medical interpreters during Cuna cohorts
  • Utilizing evaluation of the first cohort, work with leadership to improve the program for the cohorts two and three.
  • Collaborate with Centro’s Community Resources team to maintain an updated list of community resources for program participants.
  • Maintain an accurate record of all interactions with Centro’s clients through our customer management system (Salesforce), following the protocols for internal and external referrals.
  • Develop relationships with partners that support our community.
  • Facilitate Mom & Baby/Postpartum support group
  • Other duties as assigned.

Qualifications

  • Fully Bilingual in English and Spanish (written and spoken).
  • Associates Degree or Certification in health related field
  • Work Experience as a social worker, Doula, Child Birth Educator or or other related fields preferred.
  • Experience leading groups and with Group Facilitation
  • Project Management Experience
  • Corazón for Centro Hispano’s goals and mission.
  • Demonstrate the ability to organize, direct /guide, problem-solve and provide results.
  • Discretion handling sensitive and confidential information.
  • Knowledge of google suite (gmail, sheets, docs)
  • Excellent organizational skills, ability to prioritize work to meet deadlines, ability to work at a fast pace, and multitask.
  • Experience with or passion for working with culturally diverse populations as well as vulnerable populations
  • Must have a valid state driver’s license and reliable transportation.
  • Must be authorized to work in the United States.

Salary, Schedule and Benefits

  • $43,000 - 48,000 a year, dependent on education and experience
  • 40 hours per week
  • Schedule: Monday- Thursday 8:30- 5:00 pm in office, Friday flexible hours
