Patient Care Coordinator/Scheduler – Home Health
The Patient Care Coordinator assumes the responsibility to schedule patient services and to coordinate their care staffing in accordance with state and federal guidelines.
Job Qualifications
Education: Graduate of an accredited High School.
Experience: One-year experience in a clinical setting, Home Health preferred.
Skills: Good communication, interpersonal and organizational skills.
Requirement: Reliable transportation and valid auto liability insurance.
Work Environment: Works in an office environment; Noise level may be moderately high at times; The ability to work a flexible schedule and extended hours; The ability to travel locally and some exposure to inclement weather.
Job Specific Efforts Required: Periodic times of prolonged standing, sitting and walking; working under conditions necessary to meet patient’s needs and Program timeframes; timely decision making and resource acquisition; requires hand-eye coordination and manual dexterity.
Essential Functions of the Patient Care Coordinator include but are not limited to:
