ROLE:
Reporting to the Program Director, the Clinician responds to the needs of clients to maximize the recovery, resilience, and wellness of individuals who are developing or experiencing mental health, alcohol, or drug concerns.
ESSENTIAL FUNCTIONS:
Utilize knowledge of current theories, principles, practices, and relevant screening and assessment tools for providing services to clients with behavioral health care symptoms and diagnosis.
Collaborates with clients and their supporters to develop personalized, achievable goals, objectives and interventions that promote self-reliance.
Possess in-depth understanding of theory, best practice, and community resources applicable to the program’s activities and target population to provide clinical services and insure appropriate, high quality services to clients.
Synthesizes assessment information so that clearly stated, prioritized problems can be addressed by available services and resources.
Provide appropriate recommendations for care and linkage services for clients requiring on-going treatment.
Conducts thorough and comprehensive program specific assessments that identify the individual strengths and needs of each client.
Collaborate with clients and their supporters to develop personalized, achievable goals, objectives and interventions that promote self-reliance.
Teach simple and complex skills that help members gain knowledge, develop competencies, explore feelings and/or promote mutual support.
Makes referrals and provides sufficient advocacy and follow-up to ensure that clients are connected to needed services and supports both within and outside the organization. May refer to internal staff as appropriate for these functions.
Generates high quality consistent, accurate and timely documentation of service provision, including summaries of consumer progress and/or service activities.
Records meet all applicable procedures, regulatory and professional standards for timeliness, completeness, and qualitative aspects of care.
Documentation is consistent and supports billing activities.
Treatment completion rate of 65% or higher.
Communicates and collaborates with internal and external providers to ensure continuity of services and resources for intended recipients.
Plans for and provides effective, understandable, and respectful service to both staff and consumers that are compatible with an individual’s beliefs.
Provide continuity of care/follow-up services for consumers and support services for consumers, their families, and significant others.
Provides clinical supervision to students and provisional licensees as assigned.
Responsible for completing competency-based workplace safety training as assigned by the agency.
Performs other duties as assigned.
