Central Ohio Area Agency on Aging is Hiring an Aging Programs Care Coordinator (Franklin County) Near Columbus, OH

If so, consider an Aging Programs Care Coordinator position at The Central Ohio Area Agency on Aging (COAAA)!

  • Conducts home visitations for in-person assessments
  • Assesses and documents health and social environment concerns to determine needs
  • Monitors services delivered to ensure the client remains health and safety
  • Provides crisis intervention to address living conditions and medical issues
  • Maintains accurate and up-to-date case records
  • Administers pre-admission screenings, telephone interviews and desk reviews to determine program eligibility, nursing facility admission and LTC services
  • Interprets and applies program requirements for nursing facility admission or LTC services
  • May recruit and provide training, support and guidance, to individuals serving as volunteer guardians
  • Attends meetings and training sessions, as required

Minimum Qualifications:

  • Valid state of Ohio license as a Social Worker OR a valid state of Ohio license as a Registered Nurse
  • Valid driver’s license
  • Willingness to travel for in-home assessments

Knowledge, Skills, and Abilities:

  • Considerable knowledge of social work or nursing principles and practices
  • Experience with Medicare and Medicaid rules and regulations, local social services and community resources available
  • Understanding of the principles of behavioral health and gerontology
  • Basic medical terminology and pharmacology
  • Ability to administer crisis intervention techniques and conflict resolution methods
  • Demonstrates effective communication with the elderly population, individuals with physical disabilities and unstable medical conditions
  • Maintains productive working relationships
  • Highly organized and takes pride in accuracy
  • Above average computer competency
  • Awareness of personal safety and personal protective measures

The Central Ohio Area Agency on Aging (COAAA) plans, funds and delivers social services to assist adults and individuals with disabilities remain safe and independent in their homes. With the assistance of area providers, COAAA arranges and coordinates services to assist with day to day responsibilities, such as: homemaking, transportation, meal delivery and personal care. COAAA offers education and resources to caregivers, professionals and the public, in addition to advocating for programs and policies that benefit adults and individuals with disabilities. COAAA manages services for 10,000 people and funds services for an additional 20,000 adults in eight central Ohio counties.
