JOB SUMMARY: Responsible for assisting the Clinical Care Team in facilitating and maintaining an efficient flow of patients through the clinic, while ensuring quality care. Assists in coordinating and monitoring activities of the Clinical Care Team, coordination of quality patient care, lab work, administering medications and immunizations, and performing routine clinical duties. Trains new staff. Works as part of a health care team to provide comprehensive care in a non-judgmental manner. Upholds all policies and supports the mission of CNAHSI. Travel to other CNAHSI locations as needed.
PRINCIPAL DUTIES AND JOB RESPONSIBILITIES
1. Coordinates and facilitates structured communication to care team members and providers relating to patient care coordination, pre-visit planning, huddles, one-on-one communication, emails, patient contact and documentation of any services provided and/or health care related concerns/needs that have arisen since last visit.
2. Lead team meetings to communicate and address patient care and operational requirements.
3. Lead huddles emphasizing a team-based approach to care.
4. Coordinates the patient care activities of the care team.
5. Answers the telephone in a courteous and professional manner.
6. Performs all other duties as assigned and/or requested.
EDUCATION, EXPERIENCE AND CERTIFICATION/LICENSURE
Required Health Records Documentation: Proof of current Hepatitis B vaccination; negative TB skin test performed within the past 12 months or negative chest x-ray and no evidence of symptoms or completion of treatment. Proof of COVID-19 vaccination (optional). Annual Flu vaccination is required. A physical will be required if hired.
