Overview:

Job Summary:

The Associate Director of High-Risk Populations is responsible for the clinical oversight and patient care of the Medical Bridge Program, which provides acute and chronic care in areas such as respite, diversional healthcare, transitional housing, and mobile services. This individual will support the team through program development, strategic planning, and clinic operations, including optimizing care models and developing metrics that allow evaluation of the team and care. The Associate Director is also responsible for centralizing evidence-based clinical and preventative care education and ensuring all care initiatives have access to that education. This position will also help direct and clinically vet outreach initiatives, working closely with operations and other departments aligning with the strategic direction of Central Health. The Associate Director reports directly to the Director of High-Risk Populations.

Responsibilities:

Essential Functions:

-Establishes and maintains the highest quality, performance, and cost-effectiveness standards for the Bridge Medical Program by developing clinical standards of care, including care pathways, protocols, and evidence-based best practices.

-Serve as a subject matter expert for the organization on optimal care models for highly vulnerable populations undergoing transitions of care in various settings.

-Performs comprehensive patient assessments using observation, inspection, auscultation, palpation, and percussion.

-Orders or executes various tests, analyses, and diagnostic images to provide information on the patient's condition.

-Diagnose and treat diseases, disorders, and injuries based on the patient's medical history and physical assessment results; refer patients to medical specialists or other practitioners for specialized treatment.

-Prescribes medications and therapies for treatment and preventive care.

-Guides patients and their families on the procedures conducted, home care practices, and the necessity of follow-up visits.

-Oversees and guides both professional and ancillary healthcare personnel throughout the course of patient treatment.

-Addresses general and clinical inquiries from staff and patients regarding preventative health and treatments.

-Compiles patient medical data, including health history, treatment plans, and examination results, and records them in the electronic medical record.

-Reports required information, including deaths and outbreaks of contagious diseases, to governmental authorities.

-Organizes and leads monthly team meetings to foster collaboration and communication among team members.

-Collaborates with the marketing team to promote outreach and access to unique services within the community.

-Supports, leads and models the vision, mission, and values of Central Health.

-Collaborates with clinical operations and clinical leader leaders of Central Health to foster a cohesive, team-oriented environment.

-Develops strong community partnerships to support the well-being of highly vulnerable populations.

-Collaborates extensively with community health partners to enhance community engagement, aiming to improve the health and well-being of patients under the care of Central Health.

-Collaborates with clinical operations and nursing to promote excellence in medical education among all learners.

-Plans, promotes, demonstrates, recommends, and coordinates high standards of medical practice.

-Lead and motivate the medical providers under your supervision, utilizing effective medical and people performance management processes.

-Exemplifies service delivery excellence.

-Sets, updates, and exceeds essential quality and performance improvement goals, focusing on health equity, clinical outcomes, resourcefulness, workforce well-being, and community engagement.

-Serves as the champion for initiatives impacting Central Health's operations or patient care quality.

-Performs administrative duties as directed, up to 8 hours per week (if also assigned duties as clinic lead, will have up to 12 hours administrative time per week).

-Performs objective annual evaluations of all direct reports with input from the Practice Leader and Nurse Manager.

-Works closely with our community health partners to help facilitate community engagement in an effort to improve the health and well-being of the patients for whom we provide care.

Qualifications:

MINIMUM QUALIFICATIONS:

M.D. or D.O. from an accredited educational institution required

5 years of patient care experience required with at least 2 years being in a leadership/mentorship capacity

PREFERRED QUALIFICATIONS:

Experience developing unique ways to deliver healthcare in a compassionate, expedited and patient-centered manner in a mobile clinical setting

Experience developing and implementing medical policy, including that pertaining to clinical practice guidelines and care delivery for unique and vulnerable populations

Experience working with underserved populations

REQUIRED LICENSES/CERTIFICATIONS (upon hire):

MD/DO unrestricted license to practice medicine in the state of Texas

DEA License

BCLS Certification
