Job Title: Population Health Pharmacy Technician

Department: Population Health

Reports to: Director, Population Health

Employment Eligibility: Candidate has not been sanctioned or excluded from participation in federal or state healthcare programs by a federal or state law enforcement, regulatory, or licensing agency

Education Requirements: High School Diploma

Licensure Requirements: Pharmacy Technician Board Certificate registered with the Georgia Board of Pharmacy.

Work Experience: At least three (3) years pharmacy-related experience. Minimum one (1) year experience working in a retail pharmacy as a pharmacy technician

Job Summary:

Under the direct supervision of the CGHN Pharmacists, the Population Health Pharmacy Technician plays a key role on a multidisciplinary team that includes clinical pharmacists, nurse care managers, community wellness advocates, social workers, and other health professionals. The primary responsibility of this technician is to provide support to team pharmacists, patients, clinicians, and the care management team to ensure effective, safe, and cost-effective drug therapy in a population health model.

The Population Health Pharmacy Technician works closely and directly with patients to deliver a full continuum of medication adherence support by utilizing CGHN's various tools, applications, and data and analytics. They are responsible for obtaining medication refill histories, scheduling visits with a pharmacist, supporting the pharmacists to implement the patient's care plan, performing comprehensive pharmacy benefits investigations and other medication access related tasks such as processing prior authorizations, and assisting with provider communications.

CGHN is seeking an individual with a patient-first mindset, excellent problem solving and communication skills, who can thrive in an autonomous and evolving environment.

Job Specific and Unique Knowledge, Skills, and Abilities:

  • Understanding of population health concepts
  • Ability to develop strong, trusting relationships with leadership, physicians, pharmacists, and patients in the Region
  • Ability to navigate easily with computer skills: Microsoft Word, Excel, Outlook
  • Excellent communication (verbal and written) and presentation skills, to provide superb, personalized customer service and to instill confidence and to advocate for patients; ability to explain required information to patients in a comprehensible manner
  • Understanding of medication-related quality metrics for value-based care programs (Medication Adherence - Diabetes, Cholesterol and Hypertension, Statin Use in Persons with Diabetes, and Statins in Patients with Cardiovascular Disease)
  • Collaborative work style with the ability to work across different teams, areas of expertise, and adapt to ambiguous environments and clientele
  • Highly organized and self-motivated with the ability to adapt to various workspaces and work autonomously
  • Cultural sensitivity, understanding, and comfort with a wide range of social, racial and ethnic populations
  • Must practice discretion and confidentiality as position deals with highly sensitive and private data
  • Other professional skills and qualities: strong attention to detail, ability to self-direct through multitasking and prioritizing, dependability, empathy, focus on quality service, goal oriented

Job Responsibilities:

  • Build a trusting and working relationship with patients to identify strengths and barriers to medication access with the goal of supporting an individualized, patient-centered care plan
  • Develop and grow trusting relationships with pharmacists, providers, and practice clinical staff to provide the highest level of care for patients with complex needs
  • Identifying and resolving complex medication-related issues, and ability to know when to involve or escalate to managers and/or clinicians
  • Facilitate the completion of medication reconciliation activities, assist patients overcome barriers to medication adherence, conduct medication adherence outreach, and complete community resource referrals
  • Communicate and collaborate with local pharmacies to troubleshoot issues affecting medication adherence or access.
  • Utilize available databases, payer portals, and other data and analytic tools to assess patient activity/medication adherence and address barriers to care, when possible.
  • Reviews medication adherence reports to remove patients who do not meet criteria for intervention
  • Provides telephonic follow up with pharmacies to verify patients pick up medications, closing care gaps
  • Organize daily call lists in preparation for the Pharmacist
  • Provides medication adherence services utilizing prescribed workflows
  • Facilitate patients to the Pharmacist for all clinical needs
  • Participates in scheduled quality meetings.
  • Assist with special population health pharmacy initiatives and projects as needed.
  • Participate in staff meetings, in-services, and other program-specific tasks and activities as required.
