Job Summary

The Case Manager works with a specific specialty population, i.e. adults living with Severe Mental Illness (SMI), children and youth living with Severe Emotional Disturbances (SED), and/or adult or children living with Intellectual/Developmental Disabilities (I/DD), and individuals living with co-occurring substance use disorder.

Works with the client and his/her chosen support network in developing treatment and recovery plans that promote individual choice, the use of community resources, and formal and informal supports in pursuit of the individual’s desired future. Assists the client in the assignment of available agency resources to the plan and monitor those supports and services according to the plan. Services and supports are based on the principles of recovery and self-determination.

The Case Manager works under the clinical supervision of the Clinical Services Supervisor.

Essential Functions

· Coordinates quality person centered planning for clients and ensures its implementation based on the principles of recovery and trauma informed care. Ensures the implementation of self-determination principles that incorporates strengths and abilities.

· Provides outreach service to clients in their homes and in the community.

· According to person/family centered plans provides advocacy, linking, coordinating, and monitoring to assist with housing, education, transportation, employment, financial, social, psychiatric, and other clinical and community inclusive activities.

· Monitors mental and physical health status, including monitoring medication use and management.

· Provides appropriate interventions designed to stabilize and improve functioning and assists the clients to experience a positive recovery process from a severe mental illness and/or co-occurring disorder.

· Assists the clients to achieve quality of life.

· Provides assessments and crisis stabilization of clients experiencing a mental health crisis including consultation with primary care providers, other organizations, and individuals.

· Provides substance use disorder treatment interventions to clients with co-occurring disorders.

· Coordinates skills training and support in daily living skills and in activities designed to foster recovery.

· Makes referrals and facilitates access to health care and community resources and assists with discharge planning for assigned clients.

· Provides timely clinical documentation in the electronic medical record that includes amount, scope and duration of services provided in accordance with the person-centered plan.

· Complies with agency policy, MDHHS,, CARF, and Medicaid criteria for clinical documentation for all aspects of the medical record including, but not limited to, intake assessments, person-centered plans, person-centered plan reviews, releases of information, progress/activity notes, annual psycho-social assessments and discharge summaries.

· Promotes recovery and respects the choices and autonomy of clients while teaching and encouraging actions that will promote good mental and physical health.

· Participates in team meetings and other staff meetings as requested to maintain proficiency and develop competency, and meets regularly with supervisor for consultation, supervision, and resource development.

· Participates in meetings and trainings as requested to maintain proficiency and develop competency.

· Provide secondary agency day crisis coverage as assigned by supervisor or director.

· Contributes to team effort by accomplishing related results as needed.

· Performs duties and responsibilities in ways that promote goodwill, build positive relationships.

· Preserve human dignity and contribute positively to the CWN service organization.

· Actively participates in the mission and vision of CWN and practices using sensitivity to race, ethnicity and culture as well as assuring Trauma Informed Practices.

· Complies with all policies and procedures of the agency.

· The employee will complete other tasks as needed in various programs as assigned by supervisor.

Population specific Essential Duties and Responsibilities:

· Children and Youth -- completion of 24 hours of training each fiscal year that focus on issues that are specific to children and adolescents for those case managers assigned to the Children’s Diagnostic Services Team.

· ACT – Provides 24/7 crisis response coverage for ACT clients as needed.

Employee must possess computer, typing, grammar, punctuation, proofreading, transcription, phone proficiencies and excellent communication and customer relations skills to effectively interact with staff, clients, business partners and community stakeholders.

The employee will complete other tasks as needed in various programs as assigned by supervisor.

Adult CSM:

· Education, training and/or work experience in the behavioral health field with adults diagnosed with severe and persistent mental illness, co-occurring disorders and/or developmental disabilities.

· Individuals assigned to this specialty population should have the ability and training to gather and interpret motor/sensory, cognitive, psychosocial and physical health information for all adults and elderly.

· Individuals assigned to this specialty population should have the ability to gather and interpret information of the needs of the client and the range, intensity, and duration of care, habilitation, or rehabilitation required.

Children and Youth CSM:

· Education, training and/or work experience in the behavioral health field with youth and children with serious emotional disturbances.

· Individuals assigned to this specialty population should have knowledge regarding the growth and development of children and youth and its relation to understanding the range of treatment, care and services needed by clients in this specialty area.

General Position Competencies:

· Understanding and/or ability to learn the impact of trauma on persons lives and their recovery path for children through elderly adults

· Ability to communicate verbally and in writing effectively.

· Ability to meet at times and locations chosen by the client, guardian or other important persons involved in the client’s care and recovery plan.

· Possess or acquire ability to use computers in a clinical setting (or a non-clinical setting such as in the community or at a consumer’s home).

· Process skills in crisis management preferred

· Understanding of the impact of SUD and treatment and management for Co-Occurring disorders

· Understanding of the impact of chronic health conditions and treatment as well as impact on mental health issues.

· Effective problem-solving skills

· Basic reading and arithmetic skills are required.

· Excellent customer service skills are required.

· Ability to work efficiently providing quality services in a variety of settings in the community. (Primary Care, Hospital, School, Homes)

· Attention to detail and excellent organizational skills are required.

· Participates in meetings and trainings as requested to maintain proficiency and develop competency.

· The ability to demonstrate absolute discretion with confidential information is required.

· Experience with Microsoft Office applications (Word, Excel, Power Point, etc.) is highly desirable.

· Experience using office equipment (including, but not limited to, desktop telephone, computer, copier, fax machine, scanner, laser printer, and desktop calculator).

Requirements:

Qualifications, Skills, and Conditions

Knowledge, Education and Training (licenses, programs, or certificates) :

· Bachelor’s degree in Social Work or a related field required as outlined by the State of MI Medicaid Provider Manual.

· Full Michigan licensure preferred or Michigan Limited License in field of practice considered.

· Applicant must be eligible to complete supervision standards to meet full licensure with the first two years of employment.

· Full Michigan Licensure will be expected for continued employment

· Ability to obtain QMHP and/or QIDP appointment within the first year required.

· Must be a Child Mental Health Professional (CMPH) or qualify for CMPH as defined by Michigan Department of Community Health – for Children and youth CSM

Experience: A minimum of one year of progressively more responsible or expansive experience with adults living with serious mental illness, children living with SED and/or developmental/intellectual disabilities is required. Lived experiences with mental illness, developmental disabilities, and/or substance use disorders valued.

Interpersonal and Communication Skills : Frequently interpreting and translating facts and information; explaining situations and issues to people and advising them of alternative or appropriate courses of action; and/or interviewing and developing information from others on a moderate level.

Guidance/Work Direction Received : Vast majority of job duties are performed in accordance with well-defined policies, procedures and instruction, the incumbent uses some discretion in applying policies and procedures prescribed by management.

Supervisory or Managerial Responsibility : No managerial or supervisory authority.

Visual Concentration : Requires an average level of visual concentration.

Job Complexity : Regularly requires planning, multi-tasking, solving problems, analyzing situations, making decisions, or thinking creatively.

Working Conditions:

· Periodically requires physical exertion beyond that typically found in office or administrative jobs.

· Requires travel within the community to numerous locations and client homes and/or other organizations within the community

· Work is regularly carried out in mildly disagreeable conditions. Factors such as temperature, noise, or other surroundings yield an identifiable level of discomfort.

· Tasks performed produce exposure to injuries such as minor burns, cuts, abrasions, or falls beyond that of the typical office environment regularly.

Specific Requirements :

· Must have a valid Michigan’s driver’s license and a good driving record.

· Must be available to work a fully flexible schedule including evenings, weekends, and holidays.

· Must be certified in First Aid and CPR within 90 days of hire.
