Centra Health Careers is Hiring a RN CLINICAL COORDINATOR CMG Amherst, VA (FT/Days) Near Amherst, VA

Overview

The RN Clinical Coordinator leads clinical staff in a Centra Medical Group (CMG) practice. This role connects practice clinical staff with Centra and CMG Nursing Leadership in order to impact and improve quality of care and the patient experience. In addition, this position will perform nursing services with patients in partnership with providers. The practice of CMG nursing reflects the American Nurses Association's (ANA) Code of Ethics for Nurses, the Nurse Practice Act of Virginia, and the ANA Principles of Nurse Staffing.

Responsibilities

In collaboration with CMG Nursing Leadership and stakeholders, provides on-site leadership over clinical operations. Operationalizes change resulting from evidence-based practice or research; Drives improvement of patient service and workflow by using feedback, local and national study results, data, and best practices.
Directs care delivery by other healthcare professionals such as MAs, LPNs, and RNs. Oversees and creates day-to-day staffing scheduling and delegates work assignments.
Assists CMG Nursing Leadership with developing and implementing Quality Improvement Plans, developing and implementing outcome based patient care improvements, compiling quality improvement data, assessing quality of care provided, and identifying problems and solutions.
Utilizes nursing and clinical scope of practice, National Patient Safety Goals (NPSG), American Nurses Association (ANA) Principles for Nurse Staffing, and other related data, policies, practices, and regulations to lead, communicate, and collaborate with the interdisciplinary team to plan and provide patient care.
Performs physical assessments and health histories.
Establishes a compassionate environment by providing emotional, psychological and spiritual support to patients and families.
Provides preceptorship and mentorship to practice's clinical staff. Presents educational programs and delivers training materials to clinical staff, as well as on-boards and orients new clinical staff and providers. Ensures annual competencies are delivered, completed, and maintained by all clinical staff per The Joint Commission and other regulatory agencies.br>
May perform other duties as assigned or requested and job specification can be modified or updated at any time.

Qualifications

Required Education: Graduate of an accredited school of nursing
Required Certifications and Licensures: Current Virginia or Multistate Nursing Registration. Hold a current, active BLS Provider certification.

Lift/Carry up to 25 lbs: Frequently - from 1/3 to 2/3 of the time
