* Serve 12-15 patients per day, affording you the opportunity to focus more time on individualized patient care
* Thrive within a value-based care model, actively improving the quality of life for your patients
* Encounter a diverse caseload, spanning from preventative care to managing chronic conditions
* Dedicate your time to clinical care, including patient interaction and administrative tasks related to patient care
*Your Accomplishments are Highly Valued*:
* Hold an active, unrestricted NP/PA license in the appropriate state
* 3-5 years of experience as an NP or PA in geriatrics and/or primary care
* Maintain current certification as an NP or PA
* Possess or have the ability to obtain a current DEA license
* Embrace the role's patient-facing nature and participate in Humana/Senior Bridge's Tuberculosis (TB) screening program if selected

*Preferred Qualifications*:
* Familiarity with the Value-Based Care model
* Previous experience in outpatient senior care
