Job Description: SUMMARY OF POSITION This position will work alongside Project Manager to establish and maintain a cross-sector group of stakeholders, participants, service providers and agencies in order to address social determinants of health. Responsible for planning and coordinating meeting dates, form agendas, monitor action teams, and complete assessments as needed. ESSENTIAL DUTIES & RESPONSIBILITIES Will establish a cross-sector group including stakeholders, participants, services providers, state government staff, etc. that reflect target population. Work alongside Project Manager to identify/engage potential new partners through Consortium activities/outreach efforts Ensure that participates of the program are 25% of consortium membership Assist in coordination of developing and sharing resources with community partners to address social determinants of health and related impacts (e.g., mental health screening/ referral tools, smoking cessation resources) Will support parent led efforts and develop their skills so their expertise/feedback can advise the Consortium on needs/barriers, system changes, and improvements needed to address social determinants of health Plan and coordinate meeting dates, spaces, and agenda for each Consortium meeting Work alongside evaluation team to complete twice a year assessments on progress of the consortium to address strengths and needs identified by the members Use information based on assessments to create a strategic plan outing goals of the consortium Work with Consortium members to develop work groups based on the needs identified by members Work alongside work group chairs to develop strengths/needs to ensure work is moving forward to impact goals developed Must be able to travel within the grant covered areas within Tennessee. KNOWLEDGE, SKILLS & ABILITIES Effectively communicate via written, verbal, in person and virtual methods. Knowledge and understanding of MCH populations, proven skills in networking among a diverse group of agencies, and good writing, public speaking, planning, and personnel management abilities. QUALIFICATIONS Education Level Bachelor’s degree in counseling, psychology, or social work required. Associate degree in counseling, psychology, or social work plus one to two years' experience with proof of enrollment and good standing in a regionally accredited Bachelor’s degree counseling, psychology, or social work program will be accepted. Years of Experience Bilingual preferred. Certification/Licensure Must have reliable transportation, liability insurance and TN driver’s license.
